子宫内膜癌中VEGF和MMP-9蛋白的表达及临床意义

摘　　要：目的探讨VEGF和MMP-9蛋白在子宫内膜癌患者中的表达及临床意义。方法取2014年1月至2017年2月取子宫内膜癌患者100例,设为观察组。患者均行手术治疗,围术期取病灶组织与癌旁组织。选择同期进行手术治疗的子宫良性疾病患者100例,设为对照组。患者入院后完善相关检查,将围术期切除的病灶组织制备切片,采用免疫组织化学法检测不同组织VEGF和MMP-9蛋白水平,对子宫内膜癌中VEGF和MMP-9蛋白与临床病理进行单因素及多因素Logistic回归分析。结果观察组癌旁组织与对照组中VEGF和MMP-9蛋白阳性率比较无统计学意义(P>0.05);子宫内膜癌组织中VEGF和MMP-9蛋白阳性率,高于癌旁组织(P<0.05);单因素结果显示:子宫内膜癌组织VEGF和MMP-9蛋白阳性率与年龄、肿瘤大小及分化程度差异无统计学意义(P>0.05);子宫内膜癌组织VEGF和MMP-9蛋白阳性率与肿瘤侵袭深度、淋巴结转移、病理分期关系密切(P<0.05)。多因素Logistic回归分析结果表明:子宫内膜癌组织VEGF和MMP-9蛋白阳性率多与子宫内膜癌组织浸润深度、淋巴结转移及Dukes分期有关(P<0.05)。结论子宫内膜癌患者中VEGF和MMP-9蛋白呈高表达,且与组织浸润、淋巴结转移及临床分期有关,能直接参与子宫内膜癌的发生、发展。Objective To investigate the expression and clinical significance of vascular endothelial growth factor(VEGF)and matrix matalloproteinase(MMP-9)in patients with endometrial carcinoma.Methods One hundred patients with endometrial carcinoma who were admitted and treated in our hospital from January 2014 to February 2017 were enrolled as observation group,at the same time,the other 100 patients with benign uterus diseases who underwent surgical treatment were enrolled as control group.After admission,all the patients were examined completely,and the perioperative resected lesion tissues slices were prepared.Immunohistochemistry was used to detect the expression levels of VEGF and MMP-9 protein,and the examination results and clinicopathological features were analyzed by single factor and multi-factor Logistic regression analysis.Results There were no significant differences in the positive rates of VEGF and MMP-9 in paracancerous tissues of observation group and control group(P>0.05).However the positive rates of VEGF and MMP-9 in endometrial carcinoma tissues were significantly higher than those in paracancerous tissues(P<0.05).The single factor analysis results showed that the positive rates of VEGF and MMP-9 protein in endometrial carcinoma were not correlated with patient’s age,tumor size and differentiation degree of tumor(P>0.05),however,which were closely correlated to tumor invasion depth,lymph node metastasis and pathological stage(P<0.05).Moreover the multivariate Logistic regression analysis showed that the positive rates of VEGF and MMP-9 in endometrial carcinoma were closely correlated with the depth of invasion,lymph node metastasis and Dukes stage in endometrial carcinoma(P<0.05).Conclusion The overexpression of VEGF and MMP-9 exists in patients with endometrial carcinoma,which is closely correlated with the tumor tissue infiltration,lymph node metastasis and clinical stage,which can directly participate in the occurrence and development of endometrial cancer.
