摘　　要：目的:观察软性角膜接触镜(Soft contact lens,SCL)长期佩戴对角膜上皮的影响,分析其安全性。方法:取去瓣式微型角膜刀法准分子激光上皮瓣下角膜磨镶术患者术中切除的角膜皮瓣进行研究。术前框架眼镜组(n=81)、SCL组(n=72),对两组角膜健康状态及离体角膜上皮瓣厚度、角膜上皮基底细胞计数、角膜内皮细胞面积、角膜内皮细胞密度、角膜内皮细胞面积变异系数进行比较,总结SCL对角膜上皮的影响。结果:SCL组点状角膜炎、结膜炎发生率高于框架眼镜组,其离体角膜上皮瓣厚度(55.12±4.31μm)、角膜上皮基底细胞计数(3755.60±155.26)个/mm^2均低于框架眼镜组的(57.64±3.55)μm、(3995.71±342.08)个/mm^2,差异有统计学意义(P<0.05)。框架眼镜组角膜上皮细胞形态规则、排列致密,而SCL组角膜上皮细胞形态欠规则、排列疏松。结论:长期佩戴SCL可造成角膜上皮厚度及细胞数量下降,以及角膜上皮组织结构的病理学改变,可能是引发角膜受损的主要原因。Objective:The objective of this study was to observe the influence of long-term soft contact lens(SCL)wearing on corneal epithelium and to analyze its security.Methods:The corneal flap excised during laser-assisted subepithelial keratectomy was collected and analyzed.The sample was divided into framed glasses group(n=81)and SCL group(n=72).The corneal flap thickness,number of corneal epithelial basal cell,area of corneal endothelial cells,corneal endothelial cell density and variation coefficient of corneal endothelial cell area of the two groups were compared.And the influence of SCL on corneal epithelium was summarized.Results:The incidence of punctate keratitis and conjunctivitis of the SCL group was higher than that of the framed glasses group.While the vitro corneal flap thickness(55.12±4.31μm)and number of corneal epithelial basal cell(3755.60±155.26/mm2)of the SCL group were lower than those of the framed glasses group,the difference were statistically significant(P<0.05).The corneal epithelial cell in the framed glasses group was well-organized and densely arranged,while the SCL group was irregular and loose.Conclusion:Long-term soft contact lens(SCL)wearing can result in the decline in the number and the thickness of corneal epithelial cell and cause the pathologic changes of its structure,which may be the cause of cornea damage.
