摘　　要：目的探讨重症肌无力(MG)患者外周血B淋巴细胞亚群、CD80、CD86表达水平及临床意义。方法选取新发MG患者62例(MG组)及30名健康志愿者(对照组)作为研究对象,根据改良Osserman临床分型将MG组分为眼肌型MG 30例、全身型MG 32例。采用荧光标志流式细胞术检测受试者外周血中未活化B细胞(CD19^+CD27-)、记忆性B细胞(CD19^+CD27^+)以及B淋巴细胞CD80、CD86的表达水平,分析MG患者CD80、CD86的表达水平与临床绝对评分的相关性。结果 MG组CD19^+CD27-细胞百分比、CD80及CD86阳性细胞百分比均高于对照组(P<0.05),且全身型MG及眼肌型MG的CD19^+CD27-细胞百分比、CD80及CD86阳性细胞百分比均高于对照组(P<0.05)。MG患者临床绝对评分与CD80、CD86表达水平均无相关性(P>0.05)。结论 MG患者存在以CD19^+CD27-细胞比率增高为特征的B细胞亚群紊乱,同时CD80、CD86阳性细胞比率升高,提示共刺激分子CD80、CD86在MG患者B淋巴细胞活化过程中可能起着重要作用。Objective To explore the expression levels and clinical significance of peripheral blood CD80,CD86 and B lymphocyte subsets in patients with myasthenia gravis(MG).Methods A total of 62 patients with newly diagnosed MG(MG group)and 30 healthy volunteers(control group)were enrolled as subjects.The MG group was divided into ocular MG(30 cases)and systemic MG(32 cases)according to modified Osserman clinical classification.Fluorescent labeling flow cytometry was used to detect the expression levels of non-activated B cells(CD19+CD27-),memory B cells(CD19+CD27+)as well as the expression levels of B lymphocyte CD80 and CD86 in peripheral blood of the subjects,and the correlation of the CD80 and CD86 expression levels with the clinical absolute score was analyzed among patients with MG.Results The percentage of CD19+CD27-cells,as well as the percentages of positive CD80 and CD86 cells,in the MG group was higher than that in the control group(P<0.05),and patients with systemic MG or ocular MG had a higher percentage of CD19+CD27-cells and higher percentages of positive CD80 and CD86 cells compared to the control group(P<0.05).The clinical absolute score did not correlate with the expression level of CD80 or CD86 in patients with MG(P>0.05).Conclusion B lymphocyte subsets dysfunction which is characterized by increased percentage of CD19+CD27-cells,as well as elevated percentage of positive CD80 or CD86 cells,exists in patients with MG,indicating costimulators molecular CD80 and CD86 play an important role in the activation of B lymphocytes in MG patients.
