摘　　要：目的探讨丘脑出血破入脑室致脑中心疝及疝前发生的影响因素。方法选择丘脑出血破入脑室致脑中心疝及疝前的患者30例为病例组,同期丘脑出血破入脑室未发生脑疝的患者40例为对照组。收集所有患者的相关临床资料,分析丘脑出血破入脑室患者发生脑中心疝及疝前的影响因素。结果多因素分析结果显示,入院时血压、丘脑血肿量、脑室铸型、急性梗阻性脑积水为丘脑出血破入脑室患者发生脑中心疝及疝前的影响因素(P<0.05)。结论入院时血压高、脑室外血肿量>15 ml、脑室铸型、急性梗阻性脑积水是丘脑出血破入脑室患者发生脑中心疝及疝前的危险因素。Objective To investigate the influencing factors of cerebral central hernia and prehernia caused by thalamic hemorrhage ruptured into ventricle.Methods Thirty cases of cerebral central hernia and prehernia caused by thalamic hemorrhage ruptured into ventricle were enrolled as case group,and 40 concurrent patients with thalamic hemorrhage ruptured into ventricle but without cerebral hernia as control group.The related clinical data of all patients were collected,and the influencing factors of cerebral central hernia and prehernia were analyzed among patients with thalamic hemorrhage ruptured into ventricle.Results The results of multivariate analysis showed that blood pressure on admission,amount of thalamic hematoma,ventricular casting,and acute obstructive hydrocephalus were the influencing factors of the incidence of cerebral central hernia and prehernia among patients with thalamic hemorrhage ruptured into ventricle(P<0.05).Conclusion Elevated blood pressure on admission,amount of intraventricular hematoma more than 15 ml,ventricular casting,and acute obstructive hydrocephalus are the risk factors for cerebral central hernia and prehernia in patients with thalamic hemorrhage ruptured into ventricle.
