重度COPD患者与晚期肺癌患者姑息照护需求的比较研究

摘　　要：目的:比较慢性阻塞性肺疾病(COPD)患者与肺癌(LC)患者姑息照护需求,探索姑息照护需求与各症状负担之间的关系。方法:采用姑息照护结局量表(POS量表),修订版的Borg量表,KPS卡氏功能状态量表,HAD综合医院焦虑抑郁量表分别对77例COPD患者和77例肺癌患者进行比较研究。结果:本研究中COPD患者的姑息照护需求水平为(14.584±5.434)分,LC患者的姑息照护需求水平为(14.376±5.316)分;COPD患者与LC患者在姑息照护需求和焦虑抑郁方面比较,差异无统计学意义,但在呼吸困难和功能状态方面比较,差异有统计学意义;COPD患者的姑息照护需求与呼吸困难有明显相关性,LC患者的姑息照护需求与呼吸困难无明显相关性,但两组患者均与功能状态和焦虑抑郁成正相关。结论:COPD患者与LC患者的姑息照护需求相当,处于中等水平,急需进行姑息照护,今后的临床实践中应重视COPD患者的照护需求,提升患者的生活质量。Objective:To compare the palliative care needs of patients with chronic obstructive pulmonary disease(COPD)and lung cancer(LC),and explore the relationship between the palliative care needs and symptom burden.Methods:77 COPD patients and 77 LC patients were investigated by Palliative Outcome Scale(POS),Modified Borg Scale,Karnofsky Performance Status(KPS)Scale,The hospital anxiety and depression scale(HAD).Results:The palliative care needs score of COPD patients was 14.584±5.434,and the LC patients’was 14.376±5.316;No significance was found in the palliative care needs score and anxiety and depression score between COPD and LC patients,but it was found in breathlessness score and functional status score;Palliative care needs was positivly correlated with functional status,and both anxiety and depression in both groups,and it was found that Palliative care needs of COPD patients was positivly correlated with breathlessness,but it was not found in LC patients.Conclusion:Both COPD and LC patients have similar palliative care needs,which was a middle level of palliative care needs.COPD patients need palliative care urgently and more attention,it should be paid to palliative care for COPD patients to improve COPD patients’quality of life.
