远端胃癌采用胃小弯全切术与传统胃癌根治术治疗的效果对比分析

摘　　要：目的探讨远端胃癌采用胃小弯全切术与传统胃癌根治术治疗的效果。方法研究80例远端胃癌患者,随机划分为参照组与研究组,各40例,参照组采用传统胃癌根治术治疗,研究组运用胃小弯全切术,分析不同手术下的患者治疗后的并发症、生存率、复发率、淋巴清扫数量、生存质量等效果情况差异。结果在生存率上,研究组1~3年的生存率显著高于参照组,组间差异有统计学意义(P<0.05);在复发率上,研究组为17.5%,显著低于参照组32.5%,组间差异有统计学意义(P<0.05);在淋巴清扫个数上,研究组(25.9±7.4)个显著高于参照组(18.4±7.6)个,组间差异有统计学意义(P<0.05);在生存质量上,研究组(88.2±15.3)分显著高于参照组(63.5±13.6)分,组间差异有统计学意义(P<0.05);在术后并发症发生率上,研究组15.0%的比例显著低于参照组37.5%,组间差异有统计学意义(P<0.05)。结论远端胃癌采用胃小弯全切术与传统胃癌根治术治疗均可以达到一定治疗疗效,其中胃小弯全切术比传统胃癌根治术可以达到更高的淋巴清扫量,降低术后并发症发生率,提升生活质量,减少术后复发率,延长寿命,提升整体的手术效果。Objective To investigate the effect of gastric bypass and radical gastrectomy for distal gastric cancer.Methods 80 cases with distal gastric cancer patients,randomly divided into control group and research group,the 40 cases,control group with traditional gastric cancer radical treatment,the team using all of gastric small turn cut method,analysis of different complications of patients after treatment,survival,recurrence,and lymph node cleaning the difference effect of quantity and quality of life,etc.Results In the survival rate,the survival rate of the group was significantly higher than that of the control group in 1-3 years,with statistically significant difference between the groups(P<0.05).In the recurrence rate,the study group was 17.5%,significantly lower than the reference group 32.5%,and the difference between the groups was statistically significant(P<0.05).In the number of lymph node dissection,the study group was significantly higher than the reference group[(25.9±7.4)vs(18.4±7.6)],and the difference between the groups was statistically significant(P<0.05).In terms of quality of life,the study group was significantly higher than the reference group[(88.2±15.3)vs(63.5±13.6)],and the difference between the groups was statistically significant(P<0.05).In the incidence of postoperative complications,the proportion of 15.0%in the study group was significantly lower than that of the control group(37.5%),and the difference between the groups was statistically significant(P<.05).Conclusion The distal gastric cancer using the stomach is little cut method and the traditional treatment of gastric cancer radical can reach a certain treatment effect,in which all of gastric small turn cut method can achieve higher than traditional gastric cancer radical lymph node cleaning capacity,reduce the incidence of postoperative complications and improve the quality of life,reduce the postoperative recurrence rate,prolong life,improve the overall operation effect.
