肺癌患者PCDH20、VEGF蛋白和HE4表达水平与临床病理的相关性研究

摘　　要：目的探讨肺癌患者原钙黏蛋白20(PCDH20)、血管内皮生长因子(VEGF)蛋白和人附睾蛋白4(HE4)表达水平与临床病理的相关性。方法手术切除的原发性肺癌患者104例组织标本作为研究对象(肺癌组),选取其中15例肺癌周边正常肺组织作为对照组。检测2组中PCDH20 mRNA、VEGF蛋白和HE4表达水平,采用免疫组化方法对肺癌组织中PCDH20、VEGF蛋白和HE4阳性细胞标注。结果肺癌组PCDH20 mRNA表达水平低于对照组,VEGF蛋白、HE4表达水平高于对照组,差异有统计学意义(P<0.05)。2组PCDH20、VEGF蛋白和HE4阳性表达情况、阳性率比较,差异有统计学意义(P<0.05)。PCDH20在不同性别间阳性率比较差异无统计学意义(P>0.05),不同病变部位、病理分期、分化程度和淋巴结转移间阳性率比较,差异有统计学意义(P<0.05)。VEGF在不同性别、病变部位间阳性率比较差异无统计学意义(P>0.05),不同病理分期、分化程度、淋巴结转移间阳性率比较,差异有统计学意义(P<0.05)。HE4在不同性别、病变部位、分化程度、淋巴结转移间阳性率比较,差异无统计学意义(P>0.05),不同病理分期间阳性率比较差异有统计学意义(P<0.05)。通过对临床和病理因素筛选以多因素Logistic回归分析发现,病理分期、分化程度、淋巴结转移是影响PCDH20和VEGF蛋白表达的因素,病理分期是影响HE4表达的因素,差异均有统计学意义(P<0.05)。结论 PCDH20、VEGF蛋白和HE4高表达于肺癌组织,且与病理分期、分化程度、淋巴细胞转移有不同程度的相关性。Objective To explore the correlation between the expression levels of protocadherin 20(PCDH20),vascular endothelial growth factor(VEGF),human epididymal protein 4(HE4)and clinicopathological features in patients with lung cancer.Methods A total of 104 tissue specimens that were surgical excised from patients with primary lung cancer were selected as research objects(lung cancer group),in which,15 normal lung tissues surrounding in primary lung cancer were selected as control group.The expression levels of PCDH20 mRNA,VEGF and HE4 protein were detected in both groups.Moreover the positive cells of expressions of PCDH20,VEGF and HE4 proteins in lung cancer tissues were labeled by immunohistochemistry method.Results The expression levels of PCDH20 mRNA in lung cancer group were significantly lower than those in control group(P<0.05),however,the expression levels of VEGF and HE4 proteins were significantly higher than those in control group(P<0.05).There were significant differences in positive expression situation and positive rates of PCDH20,VEGF and HE4 proteins between two groups(P<0.05).However there was no significant difference in positive rate of PCDH20 between different sexes(P>0.05).However there were significant differences in positive expression rates of PCDH20 in different diseased regions,pathological stage,differentiation degree and lymph node metastasis(P<0.05).There were no significant differences in positive expression rates of VEGF protein in different sexes and different diseased regions(P>0.05),but there were significant differences in different pathological stages,differentiation degree and lymphatic node metastasis(P<0.05).The multivariate Logistic regression analysis showed that pathological stage,differentiation degree and lymph node metastasis were the factors influencing expressions of PCDH20 and VEGF proteins,and pathological stage was the factor influencing HE4 protein expression,the differences were statistically significant(P<0.05).Conclusion PCDH20,VEGF and HE4 proteins are highly expr
