局部晚期直肠癌行同步化疗后综合手术治疗的疗效分析

摘　　要：目的分析局部晚期直肠癌行同步化疗后综合手术治疗的临床疗效。方法放疗科2012年1月至2014年1月收治的80例局部晚期直肠癌患者的临床资料进行回顾性分析,依据治疗方法将患者分为同步化疗后综合手术治疗组(综合治疗组,n=40)和直接根治性手术治疗组(手术治疗组,n=40),对2组患者的临床疗效、根治性切除、保肛、肝转移、局部复发情况、不良反应发生情况、1年、2年、3年生存情况进行分析。结果综合治疗组患者的总缓解率为77.5%显著高于手术治疗组的60.0%(P<0.05),综合治疗组的根治性切除率、保肛率均显著高于手术治疗组(P<0.05),综合治疗组的肝转移率、局部复发率均显著低于手术治疗组(P<0.05),综合治疗组的1年、2年、3年生存率均显著高于手术治疗组(P<0.05)。结论局部晚期直肠癌行同步化疗后综合手术治疗临床疗效较直接根治性手术治疗好。Objective To analyze the clinical effects of comprehensive surgery therapy after concurrent chemotherapy in the treatment of locally advanced rectal cancer.Methods The clinical data of 80 patients with local advanced rectal cancer who were admitted and treated in our hospital from January 2012 to January 2014 were retrospectively analyzed.According to different herapeutic methods,these patients were divided into combined modality therapy group(comprehensive surgery therapy after concurrent chemotherapy,n=40)and direct radical surgery group(surgery treatment group,n=40).The clinical effects,radical resection,sphincter preservation,liver metastasis,local recurrences,adverse reactions,as well as 1-year,2-year and 3-year survival rate were statistically analyzed and compared between two grousp.Results The total remission rate in combined modality therapy group was 77.5%(31/40),which was significantly higher than that[60.0%(24/40)]in surgery treatment group(P<0.05).The radical resection rate,sphincter preservation rate and 1-year,2-year and 3-year survival rate in combined modality therapy group were significantly higher than those in surgery treatment group(P<0.05).However the liver metastasis rate and local recurrence rate in combined modality therapy group were significantly lower than those in surgery treatment group(P<0.05).Conclusion The therapeutic effects of comprehensive surgery therapy after concurrent chemotherapy on local advanced rectal cancer are superior to those by direct radical surgery.
