摘 要:目的探讨三结构域蛋白14(TRIM14)在人胃癌组织中的表达,分析TRIM14的表达水平与胃癌患者临床病理特征和预后的关系。方法收集手术切除的胃癌组织及对应的癌旁组织共189例,采用实时定量PCR(qRT-PCR)和免疫组化染色分别检测TRIM14mRNA及蛋白在胃癌及对应癌旁组织中的表达水平,采用卡方检验分析TRIM14mRNA表达水平与胃癌患者临床病理特征的相关性,通过单因素和多因素Cox回归模型评估TRIM14mRNA在胃癌风险评估中的作用,Kaplan-Meier生存分析TRIM14mRNA表达水平与胃癌患者无瘤生存期和总生存期的关系。结果免疫组化结果显示,胃癌组织中TRIM14阳性细胞比例显著高于癌旁组织[(95±4)%vs.(10±5)%]。TRIM14mRNA在胃癌组织中的平均表达量为(1.648±0.081),较癌旁组织平均表达量(0.942±0.067)显著升高(P=0.032)。胃癌组织中TRIM14表达水平与胃癌患者临床分期(P=0.028)、浸润深度(P=0.022)和淋巴结转移(P=0.017)显著相关,与患者性别(P=0.433)、年龄(P=0.961)、分化程度(P=0.126)及远处转移(P=0.916)无显著相关性。TRIM14mRNA表达水平为影响胃癌患者无瘤生存(RR:1.621,95%CI:1.312~2.492,P=0.037)及总生存期(RR:1.742,95%CI:1.498~2.813,P=0.033)的独立影响因子。高表达TRIM14组的胃癌患者中位无瘤生存时间显著低于低表达组(13.7月vs.20.3月,P<0.01),高表达TRIM14组中位总生存时间显著低于低表达组(29.3月vs.36.5月,P=0.030)。结论TRIM14在胃癌组织中呈高表达状态,并与胃癌恶性病理因素相关,TRIM14可能参与胃癌发生及恶性进展过程,可作为判断胃癌患者预后的重要指标。Objective To investigate the expression of tripartite motif 14(TRIM14)in gastric carcinoma(GC)tissues and its association with clinicopathological features and prognosis of GC patients.Methods Immunohistochemistry and qRT-PCR were used to detect the expression of TRIM14 mRNA and protein levels in GC tissues and matched tumor-adjacent tissues(n=189 each).Furthermore,the chi-square test was employed to examine the correlation between TRIM14 expression and clinicopathological characteristics of GC.Univariate and multivariate Cox regression analyses were used to analyze the role of TRIM14 mRNA level in the evaluation of the risk of GC,and Kaplan-Meier survival analysis to measure the relationship between the TRIM14 mRNA level and the disease-free survival and overall survival of patients.Results The percentage of TRIM14-positive cells was significantly higher in GC tissue than in matched tumor-adjacent tissues[(95±4)%vs.(10±5)%].The average expression level of TRIM14 mRNA was(1.648±0.081)in GC tissue,significantly higher than(0.942±0.067)in matched tumor-adjacent tissues(P=0.032).The expression level of TRIM14 mRNA was associated with TNM stage(P=0.028),invasion(P=0.022)and lymphatic metastasis(P=0.017).However,there was no significant association between TRIM14 mRNA expression and gender(P=0.433),age(P=0.961),degree of differentiation(P=0.126)or remote metastasis(P=0.916).TRIM14 mRNA was an independent predictive factor of disease-free survival(RR:1.621,95%CI:1.312-2.492,P=0.037)and overall survival(RR:1.742,95%CI:1.498-2.813,P=0.033).The median disease-free survival and the median overall survival were significantly lower in TRIM14 mRNA over-expression group than in TRIM14 mRNA down-expression group(median disease-free survival:13.7 months vs.20.3 months,P<0.01;median overall survival:29.3 months vs.36.5 months,P=0.030).Conclusion TRIM14 is highly expressed in GC and the overexpression of TRIM14 is associated with malignant clinicopathological features and poor prognosis of GC patients.Our results suggested tha
