推龈技术在前牙美学修复中的临床应用

摘　　要：目的:分析推龈器在前牙美学预备过程中对牙龈及修复效果的影响。方法:选择前牙美学修复患者23人共28个牙位,随机分为2组(n=14),分别采用推龈器和排龈线排开牙龈后预备龈下肩台,评价两组牙龈损伤情况、肩台位置、模型上基牙边缘完成线及修复体边缘密合性。结果:推龈器组在肩台位置控制的准确性、模型上基牙边缘完成线及牙龈损伤均优于排龈线组(P<0.05),而两组修复体的边缘密合性则无统计学差异(P>0.05)。结论:对于前牙美学修复设计龈下肩台者,推龈技术能有效控制肩台的位置,减少对牙龈的损伤,提升基牙边缘完成线制备的质量。AIM:To evaluate the clinical prosthetic effects of gingival retractor in aesthetic restoration of anterior teeth.METHODS:28 units of anterior full crown in 23 patients were randomly divided into 2 groups(n=14).The tooth shoulder was prepared with gingival retractor in GR group and that with retraction cord in RC group.The teeth in both groups were restored using Zirconia full ceramic crowns.Then the gingival injury,shoulder position,marginal adaptation and morphology of finishing line in all patients were evaluated in plaster models.RESULTS:Marginal adaptation did not show significant difference between 2 groups(P>0.05).However,GR group showed better results for of gingival injury control,shoulder position accuracy and morphology of finishing line(P<0.05).CONCLUSION:In aesthetic restoration of anterior teeth,the application of gingival retractor during sub-gingival shoulder preparation has positive impact for gingival injury control,accurate shoulder position and the quality of morphology of finishing line.
