摘 要:随着质子泵抑制剂(PPI)的长期应用,营养素缺乏、骨质疏松症、呼吸道和肠道感染、胃底腺息肉等不良反应频繁出现,近年来发现PPI也与艰难梭菌感染的风险增加相关,其导致艰难梭状杆菌感染率上升。正常情况下,艰难梭菌与肠道有益菌群处于平衡状态,若肠道正常菌群的平衡被打破,艰难梭菌的产毒株就会移居或增殖,导致假膜性肠炎,此外其致病作用还与脂质代谢和胆汁酸转化相关。PPI导致艰难梭菌感染的具体作用机制尚不明确,但肠道菌群失调在其中起到了重要作用。With the long-term use of proton pump inhibitor(PPI),adverse reactions such as nutrient deficiency,osteoporosis,respiratory and intestinal infections,stomach and stomach polyps occur frequently.Recently,the use of PPI is associated with an increased risk of infection with clostridium difficile(CD),and it increases the infection rate of CD.Under normal circumstances,CD and intestine beneficial bacteria are in a balanced state,but if the balance of the normal intestinal flora is broken,the CD toxin production will migrate or proliferate,leading to pseudomembranous enteritis,in addition,its pathogenesis is also associated with lipid metabolism and bile acid conversion.The specific mechanism of PPI causing CD infection is unclear,but it′s known that intestinal flora disorder plays an important role.
