摘 要:目的分析先天性视锥视杆细胞营养不良症(cone-rod dystrophy,CORD)患者血清脂质浓度变化情况。方法采用回顾性序列病例研究方法 ,选取临床诊断为CORD的患者50例作为CORD组,选取52位正常人作为正常对照组,按盲法由专业技术人员测量两组血清中低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、甘油三酯(triglycerides,TG)、总胆固醇(total cholesterol,TC)含量,并对两组测量结果进行比较,分析CORD患者体内血清脂质浓度的变化情况。结果 50例CORD患者中血脂水平异常者占48.00%,其中高TG血症者占54.17%,高TC血症者占12.50%,混合型高脂血症者占25.00%,低HDL-C血症者占8.33%。CORD患者中血清TG浓度[(1.874±1.745)mmol·L^(-1)]和TC浓度[(4.719±0.746)mmol·L^(-1)]与正常对照组[(0.848±0.316)mmol·L^(-1)]和[(4.098±0.596)mmol·L^(-1)]相比均显著升高,差异均有统计学意义(t=-4.171、-4.657,均为P=0.000);HDL-C浓度[(1.138±0.290)mmol·L^(-1)]与正常对照组[(1.386±0.226)mmol·L^(-1)]相比显著下降,差异均有统计学意义(t=4.817,P=0.000);LDL-C浓度[(2.579±0.541)mmol·L^(-1)]与正常对照组[(2.408±0.551)mmol·L^(-1)]相比差异无统计学意义(t=^(-1).584,P=0.116)。结论 CORD患者血清TG和TC浓度升高、HDL-C浓度降低,这可能与CORD的发病相关。Objective To analyze the serum lipids changes in patients with cone-rod dystrophy.Methods A retrospective sequence case study was conducted in 50 patients with congenital cone-rod dystrophy(CORD)who were selected as the CORD group and 52 normal subjects as the normal group.The serum levels of low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),triglycerides(TG)and total cholesterol(TC)were measured by professional technicians in hospital according to the single blind study.Data were analyzed statistically between the CORD group and normal group.Results Among the 50 patients with CORD,48.00%had abnormal blood lipid levels,of whom 54.17%were hypertrichonemia,12.50%were hypercholesterolemia,25.00%were mixed hyperlipidaemia,and low HDL-C accounted for 8.33%,respectively.The serum concentrations of TG and TC were(1.874±1.745)mmol·L-1 and(4.719±0.746)mmol·L-1 in the CORD group,which were significantly higher than those of the normal group[(0.848±0.316)mmol·L-1 and(4.098±0.596)mmol·L-1](t=-4.171,-4.657,both P=0.000),but the serum level of HDL-C was(1.138±0.290)mmol·L-1 in the CORD group,which was significantly lower than that of the normal group[(1.386±0.226)mmol·L-1](t=4.817,P=0.000).There was no significant difference in the serum level of LDL-C between CORD and normal group[(2.579±0.541)mmol·L-1 vs.(2.408±0.551)mmol·L-1](t=-1.584,P=0.116).Conclusion Elevated serum concentrations of TG and TC,and decreased HDL-C level in CORD patients may be associated with the development of CORD.
