5mL注射器经睫状体扁平部25G微切口取出硅油的临床研究

摘　　要：目的 研究5 mL注射器经睫状体扁平部25G微切口取出硅油的可行性、有效性和安全性。方法 2016年1月至2017年6月,该院因拟行硅油取出而入院的患者186例(186眼)均采用球周麻醉,用5 mL注射器经睫状体扁平部25G微切口抽吸,取出硅油,观察术中术后情况,术后随访6月。结果 硅油取出时间为9~14 min,术后低眼压6眼(3.2%),玻璃体腔及前房见血细胞4例(2.2%),术后平均最佳矫正视力均有不同程度提高。观察6个月,未见明显眼底病变加重及视网膜脱离,无并发高眼压,无感染。5例(2.7%)因白内障加重,行白内障手术。结论 5 mL注射器经睫状体扁平部25G微切口取出硅油具有简单易操作、耗时短、效率高、风险及费用低等优点,可作为有晶体眼硅油取出术的可靠术式之一,值得临床推广。Objective Study on the feasibility,efficacy and safety of silicone oil removal with a 5 mL syringe by 25G micro incisions in pars plana.Methods 186 patients(eyes)with silicone oil filled were included this study from Jan 2016 to Jun 2017 in the hospital.All eyes were received operation for silicone oil removal with a 5mL syringe by 25G incisions in pars plana under peribulbar anesthesia.Removal of silicone oil time and complications were observed.The follow-up time were 6 months.Results The silicone oil removal time was 9-14 min,hypotony 6 eyes(3.2%),blood cell in anterior chamber and vitreous cavity 4 eyes(2.2%),there was no detachment of retina and choroid detachment,no hypertonia or infection in any eye,the average corrected visual acuity was improved.5 eyes were operated for cataract became heavier.Conclusion The way of silicone oil removal with a 5 mL syringe by 25G incisions in pars plana is simple operation,short time,high efficiency,low risk and low cost.It is worth recommending for removal of silicone oil from lens eyes.
