HE4、CEA和糖类抗原肿瘤标志物检测对卵巢癌的诊断价值分析

摘　　要：目的探讨人附睾蛋白4(HE4)、癌胚抗原(CEA)、糖类抗原(CA125、CA199)等肿瘤标志物对卵巢癌的诊断价值。方法 120例卵巢癌患者纳入卵巢癌组,选取同期收治的卵巢良性病变患者112例,纳入卵巢良性病变组,选取同期健康体检者108例作为对照组。比较3组的肿瘤标志物(HE4、CEA、CA125、CA199)水平,卵巢癌组不同分期上述指标水平差异,各指标的诊断价值。结果 3组受试者HE4、CEA、CA125、CA199的水差异有统计学意义(P<0.05)。Ⅲ～Ⅳ期卵巢癌患者的HE4、CEA、CA125、CA199水平均显著高于Ⅰ～Ⅱ期(P<0.05)。联合检测的敏感度、约登指数和阴性预测值均高于各单一指标检测结果,分别为95.00%,0.70和96.51%。单一指标曲线下面积最高为HE4(0.912),最低为CA199(0.681),联合检测为0.951。结论 HE4、CEA、CA125、CA199能够辅助诊断早期卵巢癌,并且上述指标联合检测诊断价值高于单一检测价值。Objective To explore the clinical value of tumor markers including HE4,CEA,CA125 and CA199 in early diagnosis of ovarian cancer.Methods A total of 120 patients with ovarian cancer were enrolled as ovarian cancer group,112 patients with benign ovarian lesion were enrolled as benign ovarian lesion group,and 108 healthy subjects who underwent?physical examinees?were served as control group.The levels of tumor markers of the three groups,the difference of the indexes at different stages were observed and compared among the three groups.Results There were significant differences in the levels of HE4,CEA,CA125 and CA199 among the three groups(P<0.05),in which,the levels of HE4,CEA,CA125 and CA199 of patients with ovarian cancer at stageⅢ~Ⅳwere significantly higher than those at stageⅠ~Ⅱ(P<0.05).Moreover,the sensitivity,Jorden index and negative predictive value of combined detection were 95.00%,0.70,96.51%,respectively,which were significantly higher than those by single index detection.The highest AUC in single index detection was HE4(0.912),and the lowest one was CA199(0.681).AUC of combination detection was 0.951.Conclusion The detection of HE4,CEA,CA125 and CA199 is helpful to diagnose early ovarian cancer,moreover,the combination detection of these indexes is superior to the single index detection.
