摘　　要：目的探讨转铁蛋白受体1(transferrin receptor 1,TfR1)在慢性浅表性胃炎,多阶段胃癌前病变(慢性萎缩性胃炎、肠化生、不典型增生)和胃癌中的表达情况及临床意义。方法用免疫组织化学法检测TfR1在31例慢性浅表性胃炎,32例萎缩性胃炎、30例肠化生、26例非典型增生及64例胃癌中表达的变化情况,并分析TfR1表达与临床资料的关系。结果 TfR1在胃癌组织中呈高表达,且主要表达于胃癌细胞的胞膜,胞膜邻近的胞浆也有表达。在非癌组织(慢性浅表性胃炎、慢性萎缩性胃炎、肠化生和非典型增生)中TfR1表达的阳性率低于胃癌组(P<0.05)。TfR1的阳性表达与胃癌的浸润深度及是否有淋巴结转移有关(P<0.05),与胃癌患者的性别,年龄,分化程度无关(P>0.05)。结论 TfR1在胃癌组织中表达升高,且与胃癌的转移有一定的相关性。TfR1可作为胃癌诊断和治疗的新靶点。Objective To explore the expression and clinical significance of transferrin receptor 1(TfR1)in tissues of human chronic superficial gastritis,multi stage gastric precancerous lesions including chronic atrophic gastritis,intestinal metaplasia,atypical hyperplasia and gastric carcinoma.Methods The changes of TfR1 protein expression were detected by immunohistochemistry in 31 cases chronic superficial gastritis,32 cases chronic atrophic gastritis,30 cases intestinal metaplasia,26 cases atypical hyperplasia,and 64 cases gastric carcinoma.The relationship between TfR1 protein expression and the clinical data was analyzed.Results TfR1 was highly expressed in gastric carcinoma tissues,and TfR1 was mainly located in the cell membrane of gastric cancer cell and cytoplasm adjacent to cell membrane.The positive rate of TfR1 expressions in non-cancerous tissues including chronic superficial gastritis,chronic atrophic gastritis,intestinal metaplasia and atypical hyperplasia was significantly lower than that in gastric carcinoma tissues(P<0.05).The expression of TfR1 was correlated with invasion depth of gastric carcinoma and lymph node metastasis(P<0.05),however,which was not correlated with patient’s sex and age as well as the differentiation degree of tumor(P>0.05).Conclusion The expression levels of TfR1 are increased in gastric cancer tissues,which are correlated with the metastasis of gastric carcinoma at some extent,therefor,TfR1 can be regarded as an new target in diagnosis and treatment of gastric cancer.
