牛痘疫苗致炎兔皮提取物注射液治疗带状疱疹后遗神经痛的可行性观察及对脑源性神经营养因子、β-内啡肽表达的影响

摘　　要：目的探讨牛痘疫苗致炎兔皮提取物注射液在治疗带状疱疹后遗神经痛(postherpetic neuralgia,PHN)中的可行性及对脑源性神经营养因子(brain derived neurotrophic factor,BDNF)、β-内啡肽表达的影响。方法选取2016年1月—2017年9月我院收治的78例PHN患者为研究对象,按照随机数字表法分为研究组(39例)和对照组(39例),对照组给予常规治疗,研究组在常规治疗基础上给予牛痘疫苗致炎兔皮提取物注射液静脉滴注。3周后观察患者的临床疗效、疼痛程度、疼痛面积、睡眠质量及药物不良反应,ELISA法检测血清BDNF和β-内啡肽水平。结果研究组的总有效率为92.31%,高于对照组的71.79%(P<0.05)。研究组的疼痛视觉模拟评分、疼痛面积和睡眠质量匹兹堡睡眠质量指数表评分低于对照组(P均<0.05)。研究组的BDNF和β-内啡肽水平高于对照组(P均<0.05)。2组患者药物不良反应主要表现为恶心、呕吐、皮疹、头晕和胃部不适,研究组药物不良反应发生率为20.51%,与对照组的17.95%比较,差异无统计学意义(P>0.05)。结论牛痘疫苗致炎兔皮提取物注射液治疗PHN可提高血清BDNF、β-内啡肽表达水平,改善患者的疼痛程度、疼痛面积及睡眠质量,具有较好的临床疗效。Objective To study the feasibility of postherpetic neuralgia(PHN)treated by extract from rabbit skin inflamed by vaccinia virus for injection(ERSVV)and its influence on the expression of brain derived neurotrophic factor(BDNF)andβ-endorphin.Methods A total of 78 PHN patients were selected from our hospital from January 2016 to September 2017,and they were divided into research group(39 cases)and control group(39 cases)according to random number table method.The control group was given conventional treatment,and the research group was given intravenous ERSVV on the basis of conventional treatment.After 3 weeks of treatment,the clinical curative effect,severity and area of pain,sleep quality and adverse drug reactions of all patients were observed,and serum BDNF andβ-endorphin levels were detected by ELISA method.Results The total effective rate(92.31%)in research group was higher than that(71.79%)in control group(P<0.05).The visual analogue scale pain score,pain area and sleep quality pittsburgh sleep quality index score of patients in research group were lower than those in control group(P<0.05).The levels of BDNF andβ-endorphin in research group were higher than those in control group(P<0.05).The adverse drug reactions in 2 groups included nausea and vomiting,rash,dizziness and upset stomach.The incidence of adverse drug reactions in research group was 20.51%,which showed no statistical difference compared with the control group 17.95%(P>0.05).Conclusions ERSVV has good clinical curative effect in treatment of PHN.It can increase the expression levels of BDNF andβ-endorphin and improve the severity and area of pain,as well as sleep quality.
