摘  要:目的探讨挤压核、压核块技术在硬核白内障超声乳化手术中的疗效。方法采用前瞻性研究方法。将2013年1月至2016年1月在我院行硬核白内障超声乳化手术患者162例(162只眼),随机分成研究组(A组)和对照组(B组),两组均采用水平劈核技术行白内障超声乳化手术,研究组(A组)采用挤压核、压核块技术。观察术中平均超声乳化时间、平均超声能量、后囊膜破裂发生率、术后角膜水肿发生率,比较两种手术方式的优劣。结果研究组(A组)各项观察指标明显低于对照组(B组),两组之间比较差异具有统计学意义(P<0.05)。结论挤压核、压核块技术在硬核白内障超声乳化手术中,能够缩短超声乳化时间,减少能量使用,降低后囊膜破裂发生率,保护角膜内皮细胞。Objective To evaluate the efficacy of lens nucleus extrusion and oppression in phacoemulsification of hard-nucleus cataract.Methods A prospective study was conducted on 162 patients(162 eyes)with hard-nucleus cataract who received phacoemulsification between January 2013 and January 2016.They were randomly assigned into the study group(group A)and control group(group B).Both groups received phacoemulsification with horizontal nucleus fragmentation.Patients in the study group(group A)were operated with lens nucleus extrusion and oppression.Mean duration of ultrasonic emulsification,mean ultrasonic energy and the percentage of broken posterior capsule during operation,as well as the incidence of corneal edema after operation were compared.Results All these indicators were significantly lower in the study group(group A)than in the control group(group B).There are significant statistical differences between the two groups(P<0.05).Conclusions Application of lens nucleus extrusion and oppression in phacoemulsification of hard-nucleus cataract could shorten the duration of ultrasonic emulsification,decrease the energy needed,reduce the risk of broken posterior capsules and protect corneal endothelial cells.
