血清α-L-岩藻糖苷酶、甲胎蛋白、脱γ-羧基异常凝血酶原检测在原发性肝癌诊断中的价值及临床意义研究

摘　　要：目的探究血清α-L-岩藻糖苷酶(AFU)、甲胎蛋白(AFP)、脱γ-羧基异常凝血酶原(DCP)检测在原发性肝癌(PHC)诊断中的价值及临床意义。方法回顾性分析我院98例PHC患者(A组)、84例良性肝病患者(B组)及84例体检健康者(C组)临床资料。记录三组血清AFU、AFP及DCP水平差异,比较三组单项及联合检测阳性率差异;并记录A组血清学联合检测检出率与超声、核磁共振成像(MRI)检出率差异。结果 A组血清AFU、AFP、DCP水平、指标单项检测及联合检测阳性率均高于B组及C组(P均<0.05),B组上述指标也高于C组(P均<0.05);A组AFU、AFP、DCP及3项联合检测阳性率分别为83.67%、72.45%、79.59%及93.88%,其中3项联合检测阳性率最高。A组血清学联合检测检出率明显高于超声及MRI检测(P<0.05)。结论 AFU、AFP及DCP联合检测可提高PHC检出率,于PHC的早期诊断有利。Objective To explore the values and clinical significance of serum alpha-L-fucosidase(AFU),alpha fetoprotein(AFP)and des-γ-carboxy prothrombin(DCP)tests in the diagnosis of primary hepatocellular carcinoma(PHC).Methods The clinical data of 98 cases of PHC patients(the group A),84 cases with benign liver disease(the group B)and 84 cases of healthy controls(the group C)in our hospital were retrospectively analyzed.The levels of serum AFU,AFP and DCP were recorded among the three groups,and the positive rates of single detection and combined detection were compared among the three groups.The difference between the serological combined detection rate of A group and the detection rate of ultrasound and magnetic resonance imaging(MRI)were also recorded.Results The serum levels of AFU,AFP and DCP,and the positive rates of single detection and combined detection in the group A were significantly higher than those in the group B and the group C(all P<0.05),and the above indexes in the group B were also significantly higher than those in the group C(all P<0.05).The positive rates of AFU,AFP,DCP and three combined tests were 83.67%,72.45%,79.59%and 93.88%respectively in the group A,and the positive rate of three combined tests were the highest.The detection rate of combined serological detection in the group A was significantly higher than that of ultrasound and MRI(P<0.05).Conclusion AFU,AFP and DCP combined detection can increase the detection rate of PHC,and it is favorable for the early diagnosis of PHC.
