化瘀涤痰汤治疗中风后遗症疗效及对患者下肢功能的影响

摘　　要：目的:探讨中药化瘀涤痰汤治疗中风后遗症的临床疗效及对下肢功能的影响。方法:选择60例中风后遗症患者为研究对象,采用随机数字表法将患者分为对照组和观察组各30例。两组患者均采取个体化基础性治疗,对照组采用口服阿司匹林联合氯吡格雷治疗,观察组给予中药化瘀涤痰汤进行治疗,比较两组患者临床疗效及下肢功能Berg平衡量表评分情况。结果:治疗后2、3、4周,两组患者中医证候积分均明显下降,且观察组比对照组下降更明显(P<0.05)。两组治疗后Berg平衡量表评分均显著升高,对照组(t=9.572,P<0.01),观察组(t=15.373,P<0.01);观察组治疗后Berg平衡量表评分明显高于对照组(t=2.307,P<0.05)。治疗后,与对照组相比,观察组患者下肢FMA评分升高(P<0.05)。观察组总有效率显著高于对照组(χ~2=4.043,P<0.05)。治疗后6个月,观察组患者身体功能、认知功能、角色功能、情绪功能及社会功能评分均显著高于对照组(P<0.05)。结论:中药化瘀涤痰汤能够有效改善中风后遗症患者的下肢运动和平衡功能,提高临床疗效,并改善患者生活质量。Objective:To investigate the clinical efficacy of Huayu Ditan decoction in treating stroke sequela and its effect on lower limb function.Methods:60 patients with apoplexy sequelae admitted were selected as the research subjects.The patients were divided into control group and observation group by random number table grouping method,30 cases in each group.The two groups were treated with individualized basic treatment.The control group was treated with oral aspirin combined with clopidogrel,and the observation group was treated with Chinese medicine Huayu Ditan decoction.The clinical efficacy and the Berg balance scale of the lower extremities were compared in the two groups.Results:At 2,3 and 4 weeks after treatment,the scores of TCM syndrome in the two groups decreased significantly,and the observation group decreased more significantly than the control group(P<0.05).After treatment,the score of Berg balance was significantly increased in the two groups,in the control group(t=9.572,P<0.01),and in the observation group(t=15.373,P<0.01),and the score of the Berg balance scale in the observation group was significantly higher than that of the control group(t=2.307,P<0.05).After treatment,compared with the control group,the FMA score of the lower extremities in the observation group increased(P<0.05).The total effective rate of the observation group was significantly higher than that of the control group(χ2=4.043,P<0.05).After 6 months of treatment,the scores of body function,cognitive function,role function,emotional function and social function of the observation group were significantly higher than those of the control group(P<0.05).Conclusion:The Chinese medicine Huayu Ditan decoction can effectively improve the lower limb movement and balance function of the sequelae of apoplexy,improve the clinical effect,improve the quality of life of the patients.
