摘 要:目的探讨膀胱尿路上皮癌微环境中CD4^+CD25^+Treg细胞与临床病理之间的关系,探讨其在膀胱尿路上皮癌进展复发及预后中的价值。方法通过免疫组织化学Elivision法检测50例膀胱尿路上皮癌组织及20例正常膀胱组织中CD4^+CD25^+Treg细胞的数量分布情况,分析膀胱尿路上皮癌微环境中CD4^+CD25^+Treg细胞分布情况与各项临床病理之间的关系。结果 CD4^+CD25^+Treg细胞在膀胱尿路上皮癌微环境中均见分布,在正常膀胱组织中未见分布。CD4^+CD25^+Treg细胞在膀胱尿路上皮癌微环境中的分布与病理分级呈显著正相关(r=0.917),差异有统计学意义(P<0.05);与淋巴结转移呈显著正相关(r=0.723),差异有统计学意义(P<0.05);与进展复发呈显著正相关(r=0.670),差异有统计学意义(P<0.05)。CD4^+CD25^+Treg细胞在膀胱尿路上皮癌微环境中的分布与患者的年龄、性别、肿瘤大小及肿瘤个数之间的差异无统计学意义(P>0.05)。结论膀胱尿路上皮癌微环境中CD4^+CD25^+Treg细胞的分布情况与其病理分级、淋巴结转移及进展复发均表现为显著正相关。膀胱尿路上皮癌微环境中CD4^+CD25^+Treg细胞分布数量增加与肿瘤的进展复发及预后具有紧密联系,CD4^+CD25^+Treg细胞可能促进膀胱尿路上皮癌的发生发展。CD4^+CD25^+Treg细胞或许能够成为一个潜在的重要指标用来判断膀胱尿路上皮癌的进展复发及预后情况。Objective To investigate the relationship between CD4^+CD25^+Treg cells and clinicopathology in the microenvironment of bladder urothelial carcinoma,and to explore its predictor value in the recurrence and prognosis of bladder urothelial carcinoma.Methods The distribution of CD4^+CD25^+Treg cells in 50 cases of bladder urothelial carcinoma tissue and 20 cases of normal bladder tissue were tested by Elivision method,and the relationship between the CD4^+CD25^+Treg cells distribution in the microenvironment of bladder urothelial carcinoma and clinicopathology were analyzed.Results CD4^+CD25^+Treg cells were found in the microenvironment of bladder urothelial carcinoma and were not distributed in normal bladder tissues.The distribution of CD4^+CD25^+Treg cells in the microenvironment of bladder urothelial carcinoma was significantly positively correlated with the pathological grading(r=0.917),and the difference was statistically significant(P<0.05).There was significant positive correlation with lymph node metastasis(r=0.723),and the difference was statistically significant(P<0.05).There was a significant positive correlation with the progression recurrence(r=0.670),and the difference was statistically significant(P<0.05).The distribution of CD4+CD25+Treg cells in the microenvironment of bladder urothelial carcinoma was not statistically significant with the age,sex,tumor size or tumor number(P>0.05).Conclusion The distribution of CD4^+CD25^+Treg cells in the microenvironment of bladder urothelial carcinoma was significantly positively correlated with pathological grading,lymph node metastasis and progression recurrence.The increase of CD4^+CD25^+Treg cell distribution in the microenvironment of bladder urothelial carcinoma is closely related to the tumor progression and prognosis,so CD4^+CD25^+Treg cells may promote the development of bladder urothelial carcinoma.CD4^+CD25^+Treg cells may be a potentially important indicator to determine the recurrence and prognosis of bladder urothelial carcinoma.
