摘 要:目的通过X线片分析泸州地区恒牙列早期安氏Ⅱ类1分类错(牙合)畸形女性儿童的第三磨牙(M3)的发育状况。方法选取两年内在西南医科大学附属口腔医院就诊的12岁恒牙列早期安氏Ⅱ类1分类错(牙合)女性儿童393例,通过分析其曲面断层片和头颅定位侧位片,按不同垂直骨面型进行定性、定量分析M3的发育状况。结果 M3的发育状况为大部分牙冠已钙化,呈帽状,Demirjian分期多处于D期(48.5%)和C期(21.1%);上颌M3与第二磨牙(M2)的夹角多为负角,下颌M3与M2的夹角多为正角,左右侧无统计学差异;上下颌M3近远中径明显大于M3的萌出间隙。结论实验对象中以髙角型病例居多,低角型病例最少。不同垂直骨面型表征类似,下颌M3较上颌M3发育略早。上颌和下颌M3分别以远中向阻生和近中向阻生为主。不同垂直骨面型的M3的阻生均以下颌多见。Objective Analysis of the development of third molar(M3)in the early stage of permanent dentition in Luzhou area by X ray analysis of the children with class 1 malocclusion.Methods 393 cases of 12 years old female children were diagnosed as classⅡ,division 1 malocclusion in permanent dentition were selected from Stomatological Hospital of Southwest Medical University.Through the analysis of orthopantpmpgraphy and cephalometry,this cases were grouped by different vertical bone types,the development of the third molar in the maxilla and mandible were analyzed,then the development status of he third molar was classified statistically according to the results.Results The development of the third molar in the sample was that most of the crown had calcification,the crown was cap-shaped,and the Demirjian stage is mostly in C phase(21.1%)and D phase(48.5%).The angle between the maxillary third molar and the maxillary second molar was negative angle,the angle of the mandibular third molar and the mandibular second molar was positive angle,and the angle of the left side and right side had no significant difference.Then the length of the third molar were significantly bigger than eruption space of the third molar.Conclusion Among the cases of female children diagnosed as classⅡ,division 1 malocclusion in permanent dentition,the majority of the cases were high-angle,the low-angle were the least.The result of different vertical bone types were similar,in which most crown of the third molar had calcification,and the mandibular third molar developed earlier than the maxillary third molar.The maxillary third molar is mainly distal impacted,and the mandibular third molars are mainly mesial impacted.The impacted third molar is mainly in mandibulary.
