肛门Soave改良术与经脐腹腔镜结肠拖出术治疗先天性巨结肠的疗效对比

摘　　要：目的:对比肛门Soave改良术与经脐腹腔镜结肠拖出术治疗先天性巨结肠的临床效果。方法:纳入76例先天性巨结肠患儿为研究对象,采用随机数字表法将其分为观察组与对照组(每组38例),对照组予以经脐腹腔镜结肠拖出术治疗,观察组予以肛门Soave改良术治疗,观察两组患儿的手术情况、治疗后肛门功能情况及排便情况。结果:观察组肛门功能总优良率(84.21%)明显高于对照组(68.42%)(P<0.05),观察组手术时间、肠蠕动恢复时间及住院时间明显短于对照组,术中出血量明显少于对照组(P<0.05),且观察组自主排便率(76.32%)明显高于对照组(57.89%)(P<0.05),观察组便秘率(7.89%)和污粪率(15.79%)均明显低于对照组的便秘率(13.16%)和污粪率(28.95%)(P<0.05)。结论:肛门Soave改良术治疗先天性巨结肠能明显改善患儿肛门功能,且手术时间、肠蠕动恢复时间及住院短,术中出血量少。Objective:To compare clinical effects of modified anal Soave surgery and transumbilical laparoscopic colon pull-through in treatment of congenital megacolon.Methods:A total of 76 children with congenital megacolon were included in the study.These patients were divided into observation and control groups according to the random number table method(38 cases in each).The control group was treated by transumbilical laparoscopic colon pull-through,while the observation group was treated with modified anal Soave surgery.The anal function,operation status and defecation of the two groups were observed.Results:The total excellent and good rate of anal function in the observation group(84.21%)was significantly higher than that in the control group(68.42%)(P<0.05).The operation time,recovery time of bowel movements,and hospitalization time in the observation group were significantly shorter than those in the control group,and the blood loss was less than that in the control group(P<0.05).Further,the rate of spontaneous defecation in the observation group(76.32%)was significantly higher than the control group(57.89%)(P<0.05);however,the constipation rate(7.89%)and defile feces rate in the observation group(15.79%)were significantly lower than(13.16%)and(28.95%)in the control group(P<0.05).Conclusions:Modified anal Soave surgery in the treatment of congenital megacolon can significantly improve the anal function of the children with less operation time,recovery time of bowel movements,short hospitalization time,and less intraoperative blood loss.
