摘 要:目的采用层次分析法(AHP)建立影响临床医学专业学位研究生临床能力培训质量的评价指标体系,结合卫计委相关政策要求,探索并实施科学、有效的质量控制策略,以提高临床医学专业学位研究生临床能力培训质量。方法结合文献检索法和专家评议确定评价指标(目的层、准则层和指标层),随机选取50名学习及规培时间超过半年以上的临床医学专业学位研究生对评价指标进行评议,采用AHP法建立评价指标体系并观察各指标权重。结果确定临床医学专业学位研究生课程体系、培训制度、临床技能考核、指导教师、培训内容、质量监控体系、组织管理、相关措施、基地建设、基地条件10个维度下属的35个指标的临床医学专业学位研究生临床能力培训质量评价指标体系。35个指标中,带教教师工作积极性占得权重最大,之后权重较大的是带教教师学习频次,导师重视程度,教学督导以及床旁病例教学。其中,岗前培训,专管生活人员,人事管理制度所占权重较小。从此评价指标体系中可以看出,评价临床医学专业学位研究生临床能力培训质量时更应该看重于指导教师,课程体系,培训内容以及质量监控体系4个维度。结论高质量临床医学专业学位研究生临床能力的培养是研究生再教育的培养目标之一,可以满足临床医疗工作专业人才需求,可以提高临床队伍的整体质量。本研究从临床能力培养的多角度着手,确定影响临床医学专业学位研究生临床能力培养的主要因素,为临床教学改革提供应对策略和方向。Objectives Try to use analytic hierarchy process(AHP)method to establish evaluation index system of the quality of clinical ability training in clinical masters,combing with the relevant policies and health planning commission requirements,in order to realize the improvement of clinical masters training quality.Methods Using the analytic hierarchy process combined with the method of consulting literature materials and experts,determine the evaluation index and index weight to establish the evaluation index system under 50 clinical masters who were in clinical study and training for more than six months.Results Establish ten dimensions evaluation system,including course system,training content,clinical skills assessment,instructor,training system,quality control system,organization and management,related measures,base construction,base conditions,with 35 secondary indexes of clinical ability training quality evaluation index system of clinical ability training of clinical masters.In the 35 indexes,the instructors work enthusiasm has the largest weight,followed by the learn frequency of instructors,tutor attention,teaching supervision and bedside case teaching.Pre-job training,specialized life staff,personnel management system have a smaller weight.It can be seen from the evaluation index system that when evaluating the quality of clinical professional training,more attention should be paid to the four dimensions of instructor,course system,training content,quality control system.Conclusion High quality clinical ability of clinical Master is one of the training objectives of postgraduate continuing education,which can meet the demand of clinical medical work,and improve the overall quality of the clinical team.This study,from the perspective of clinical competence training,determines the mainly affect factors that the clinical competence training of clinical masters,and provides coping strategies and direction for teaching reform.
