摘 要:目的研究分析血清视黄醇结合蛋白(retinol binding protein,RBP)、前白蛋白(prealbumin,PA)、总胆汁酸(total bile acid,TBA)、胆碱酯酶(cholineesterase,CHE)检测在肝脏疾病中的临床价值。方法选择2015年1月至2017年12月期间在本院就诊的200例肝脏疾病患者与在本院体检的80例健康志愿者作为研究对象,分别设为肝病组、对照组。对肝病组与对照组实施血液生化检验,测定血清RBP、PA、TBA、CHE、谷丙转氨酶(alanine aminotransferase,ALT)、谷草转氨酶(aspartate aminotransferase,AST),比较肝病组与对照组的各项血液生化检测指标,并比较不同类型肝脏疾病患者的各项血液生化检测指标。采用皮尔逊相关系数分析法分析各项血液生化检测指标与肝脏疾病的相关性。结果肝病组的RBP、PA、CHE均低于对照组(P<0.05),其TBA、ALT、AST均高于对照组(P<0.05)。急性肝炎组、慢性肝炎组、肝硬化组、肝癌组的RBP、PA、TBA、CHE、ALT、AST比较,差异均有统计学意义(P<0.05)。经相关性分析,RBP、PA、CHE与肝脏疾病呈负相关,TBA、ALT、AST与肝脏疾病呈正相关。结论 RBP、PA、TBA、CHE与肝功能指标进行联合检测,可对肝脏疾病诊断、疗效评估、预后判断具有一定的指导意义。Objective To study and analyze the clinical value of serum RBP,PA,TBA and CHE in liver diseases.Methods 200 liver diseases patients in our hospital from January 2015 to December 2017 and 80 healthy volunteers in our hospital were selected as the subjects,they were divided into liver disease group and control group.Blood biochemical tests were performed in the liver disease group and the control group,serum RBP,PA,TBA,CHE,ALT and AST were measured,the blood biochemical indexes of the liver disease group and the control group were compared,and the blood biochemical indexes of patients with different types of liver diseases were compared.The pearson correlation coefficient analysis was used to analyze the correlation between the biochemical indexes of blood and the liver disease.Results The RBP,PA and CHE of the liver disease group were lower than those of the control group(P<0.05),and the TBA,ALT and AST were all higher than those of the control group(P<0.05).The differences of RBP,PA,TBA,CHE,ALT and AST in acute hepatitis,chronic hepatitis,liver cirrhosis and liver cancer were statistically significant(P<0.05).The correlation analysis showed that RBP,PA and CHE were negatively correlated with liver diseases,and TBA,CHE,ALT and AST were positively related to liver diseases.Conclusion Combined detection of RBP,PA,TBA,CHE and liver function indicators can be of guiding significance for liver disease diagnosis,curative effect evaluation and prognosis judgement.
