姑息性手术联合区域性动脉灌注化疗对晚期胰头癌的临床观察

摘　　要：目的探讨姑息性手术联合区域性动脉灌注化疗对晚期胰头癌的临床治疗效果。方法选取2000年1月~2011年1月收治的不可根治性切除晚期胰头癌疾病患者共105例,采用随机数字表法将所有患者分为对照组(n=50)和观察组(n=55)。对照组实施常规药物对症治疗,观察组实施姑息性手术联合动脉灌注化疗治疗,比较患者术后近期和远期的生存率,对患者的生存质量进行调查。结果观察组患者术后1年、3年和5年的生存率均显著高于对照组,差异均具统计学意义(P<0.05);观察组患者整体生存质量评分显著高于对照组(P<0.05);观察组患者发生药物不良反应率与对照组比较,组间差异无统计学意义(P>0.05)。结论对于无法根治性切除的胰头癌患者实施姑息性手术联合动脉灌注化疗,能明显提高患者的生存率,对于提高患者生命质量和预后有着重要的作用,并且患者均未出现严重不良反应,治疗方案安全可靠。Objective To investigate the effect of palliative surgery combined with regional arterial perfusion chemotherapy on advanced pancreatic head carcinoma.Methods A total of 105 patients with advanced pancreatic head cancer who underwent radical resection in our hospital from January 2000 to January 2011 were selected and divided into control group(n=50)and observation group(n=55)by digital voting.The control group was treated with conventional drug therapy,and the observation group was treated with palliative surgery combined with arterial care for chemotherapy.The short-term and long-term survival rates of the patients were compared,and the quality of life of the patients were investigated.Results The survival rate of the patients in the observation group was significantly higher than that of the control group(P<0.05),and the overall survival quality score of the patients in the observation group was significantly higher than that of the control group(P<0.05).There was no significant difference between the rate of adverse drug reaction in the observation group and the control group(P>0.05).Conclusion Palliative surgery combined with arterial perfusion chemotherapy can significantly improve the survival rate of patients with pancreatic head cancer without radical resection.It plays an important role in improving the quality of life and prognosis of patients.
