摘 要:目的探讨超声引导下经皮微波消融治疗小肝癌的疗效和安全性。方法分析我院自2012年4月—2017年4月,采用超声引导下经皮微波消融术,配合术中实时超声造影、人工腹水和无水酒精注射等辅助技术,治疗高危部位小肝癌21例,分析治疗的安全性和有效性。结果围手术期出现脓毒血症1例,反应性右胸腔积液1例,未出现其它严重并发症。术中行人工腹水3例,实时超声造影发现未完全消融即刻行补充消融2例。微波消融术后1个月复查,21个病灶,有20个完全灭活,完全消融率95.2%。随诊12~67个月,1年生存率90.5%。结论超声引导下、经皮微波消融治疗高危部位小肝癌近期疗效确切,与外科手术效果相当,且因为微创,安全性较好。Objective To investigate the efficacy and safety of ultrasound guided percutaneous microwave ablation for small hepatocellular carcinoma.Methods The safety and effectiveness of the treatment were analyzed from April 2012 to April 2017,21 cases of small hepatocellular carcinoma at high risk were treated with ultrasound guided percutaneous microwave ablation,combined with intraoperative real-time ultrasound,artificial ascites and anhydrous alcohol injection.Results Perioperative sepsis occurred in 1 cases,reactive right pleural effusion in 1 cases,and no other serious complications occurred.Intraoperative ascites was performed in 3 patients,and reablate 2 cases of incomplete ablation found by ultrasound contrast.A months after microwave ablation,20 lesions were completely inactivated in 21 cases,and the total ablation rate was 95.2%.Postoperative follow-up was 12~67 months and 1 year survival rate up to 90.5%.Conclusion Ultrasound-guided percutaneous microwave ablation for the treatment of small hepatocellular carcinoma at high risk is accurate and effective.
