空肠P袢代胃术治疗胃底贲门癌40例

摘　　要：目的:探讨空肠P袢代胃术重建消化道对胃底贲门癌患者生活质量和并发症发生率的影响。方法:选取80例胃底贲门癌患者,采用随机数字表法分为研究组和对照组各40例,对照组采用标准Roux-en-Y吻合术,研究组采用空肠P袢代胃吻合术治疗,对两组术中术后及并发症发生情况进行统计分析。结果:和对照组消化道排空时间[(38±18)min]、饮食次数[(5.6±2.3)次]、饮食量[(265±38)m L]相比,研究组消化道排空时间[(60±22)min]显著较长(P<0.05),每天饮食次数[(4.3±1.2)次]显著较少(P<0.05),每次饮食量[(300±42)m L]显著增多(P<0.05),生活质量评分显著提高(P<0.05)。研究组术后并发症发生率17.5%显著低于对照组30.0%(P<0.05)。结论:空肠P袢代胃术重建消化道,能够提高胃底贲门癌患者生活质量,降低并发症发生率。Objective To investigate the influences of P jejunal loop generations stomach surgery reconstruction of digestive tract on quality of life and morbidity of patients with gastric cardia cancer.Methods Eighty cases with gastric cardia cancer were randomly selected and divided into two groups by random digital table method,the study group(n=40)and control group(n=40).The patients in control group were given standard Roux-en-Y anastomosis surgical treatment,while the patients in study group were given P jejunal loop substitute for stomach anastomosis surgical treatment,and then the intraoperative and postoperative conditions and complications of the two groups were analyzed statistically.Results In the control group,the gastrointestinal emptying time was(38±18)min,number of daily diet was(5.6±2.3)times and diet amount was(265±38)mL.In the study group,the gastrointestinal emptying time[(60±22)min]and diet amount[(300±42)mL]were significantly longer(P<0.05),the number of daily diet[(4.3±1.2)times]was significantly less(P<0.05),the quality of life score was significantly higher than that in the control group(P<0.05),the complication rate after surgery(17.5%)was significantly lower than that of the control group(30.0%)(P<0.05).Conclusion P jejunal loop generations stomach surgery reconstruction of digestive tract can effectively improve the quality of life and reduce the incidence of complications of gastric cardia cancer patients.
