隆突性皮肤纤维肉瘤的影像学表现及临床病理分析

摘　　要：目的探讨隆突性皮肤纤维肉瘤(DFSP)的影像学表现及临床病理特征。资料与方法回顾性分析经手术病理证实的11例DFSP患者的临床、影像学资料及病理结果,其中5例行CT检查(增强1例),6例行MRI检查(增强4例)。分析DFSP的临床、影像和病理特征。结果病灶位于躯干4例,乳腺2例,四肢2例,头皮2例,颈部左耳下1例;单发7例,多发4例。11例患者共18枚病灶。9例患者15枚病灶中可见悬吊征,11例可见皮肤尾征,5例见脂肪尾征。5例见多结节征和子结节外突征,其中3例多发病例10枚病灶中8枚可见多结节征和子结节外突征;3例见周围卫星征,其中2例为多发病灶。免疫组化CD34均呈阳性。结论 DFSP表现为皮肤及皮下软组织肿瘤,悬挂征、皮肤尾征、多发者呈丘状相连;伴多结节征、子结节外突征和周围卫星征是其特征性影像学表现。Purpose To explore the imaging features and clinical pathology characteristics of dermatofibrosarcoma protuberans(DFSP).Materials and Methods The clinical and imaging data and pathological findings of 11 patients with DFSP confirmed by operation and pathology were retrospectively analyzed.CT examination was performed in 5 cases(including 1 enhanced)and MRI was performed in 6 cases(including 4 enhanced).The clinical,imaging and pathological characteristics of DFSP were analyzed.Results The anatomic locations of the 11 patients included trunk(n=4),breast(n=2),limbs(n=2),scalp(n=2)and neck under left ear(n=1).There were 7 cases of single tumor and 4 cases of multiple tumors.A total of 18 lesions were investigated.Among the 11 patients,9 cases(15 lesions)showed“suspension sign”,11 cases showed“skin tail sign”and 5 cases showed“fat tail sign”,5 cases(including 3 multiple cases)showed“multi-nodular sign”and“sub-nodules outward sign”(8 lesions out of 10),and 3 cases(including 2 multiple cases)showed“peripheral satellite sign”.The immunohistochemical reports of all cases were positive for CD34.Conclusion DFSP is characterized by cutaneous and subcutaneous soft tissue tumor,with“suspension sign”,“skin tail sign”and multiple tumor showing hillock connection.“Multi-nodular sign”,“sub-nodules outward sign”and“peripheral satellite sign”are its typical imaging findings.
