P300、MMN在诊治轻度认知功能障碍和阿尔茨海默病中的应用价值

摘　　要：目的分析MCI、AD患者认知功能及事件相关电位(P300、MMN)的变化特点,探讨两者之间的关系,为MCI和AD早期诊断提供客观依据。方法选取32名认知功能障碍患者,将患者分为MCI、AD两组,并选取21名健康对照组(NC),分别采集4 h脑电及P300、MMN相关数据;使用SPSS 17. 0统计学软件,进行单因素方差分析和相关性分析,P <0. 05为差异具有统计学意义。结果 AD组P300波幅降低,潜伏期延长;且AD组患者的P300波幅、潜伏期与神经心理学测试存在显著相关性。结论 P300作为一项可靠的指标,能较为灵敏、客观的评估认知功能,其敏感性高(其潜伏期敏感性高于波幅),特异性较低;与视频脑电图相比,P300对于AD诊断更为敏感,尤其是痴呆早期患者。Objective The purpose of the present study was to analysis the cognitive function and the features of event-related potential(P300 and MMN),and to explore the relationship between them.Providing objective foundation for diagnosing MCI and AD at the early stage.Methods Collegting 32 patients were divided into two groups:MCI and AD,choosing 21 healthy elderly control group.Collecting the results of 4 h video-electroencephalogram(VEEG)and the related data of P300 and MMN.Using SPSS 17.0 software,analysis of variance(ANOVA)was used to compare in multi-group variables,and Spearman correlation analysis was used to analyze the correlation between P300,MMN and neuralpsychology test.P<0.05 was statically significant difference.Results In AD group the amplitudes of the P300 component was decreased and the latency of P300 was prolonged.Moreover AD patients exhibited significant correlation between the latencies of P300,the amplitude of P300 and neuralpsychology test.Conclusion P300 as a reliable indicator can be more sensitive and objective to evaluate the degree of the cognition function,with high sensitivity and low specificity.The P300 latency is more sensitive than the amplitude in the diagnosis of AD.Compared with video-EEG,P300 is more sensitive in the diagnosis of AD,especially at the early stage.
