摘 要:目的:探讨托毒生肌凝胶对大鼠溃疡创面的治疗作用及机制。方法:取56只SD大鼠建立全层皮肤开放性缺损创面,随机取42只建立大鼠皮肤溃疡创面模型,并随机分为三组(n=14):模型对照组(使用空白凝胶)、贝复新组(使用重组牛碱性成纤维细胞生长因子凝胶)、托毒生肌方组(使用托毒生肌凝胶)。剩余14只大鼠为对照组。观察各组创面恢复情况。计算模型大鼠治疗第7天、15天创面新生毛细血管数及成纤维细胞数。用反转录聚合酶链反应法(RT-PCR)检测各组模型大鼠治疗第7天、15天创面血管生成素-1(Ang-1)、血管内皮生长因子(VEGF)、低氧诱导因子(HIF-1α)mRNA的表达情况。结果:托毒生肌组3d、7d、15d愈合率[(41.37±16.71)%、(57.68±7.29)%、(87.29±5.74)%]明显高于模型对照组[(21.25±19.12)%、(42.21±7.24)%、(52.18±6.28)%]。托毒生肌组创面愈合时间(16.35±2.75)d明显低于模型对照组(25.22±5.37)d及贝复新组(20.72±4.25)d,(P<0.05);托毒生肌组第7天、第15天新生毛细血管[(9.47±1.50)个、(4.12±1.29)个]及成纤维细胞数目[(63.31±5.28)个、(27.98±6.03)个]与模型对照组[(7.18±1.90)个、(6.41±1.16)个、(51.77±6.01)个、(39.76±7.61)个]比较,差异均有统计学意义(P<0.05)。托毒生肌组第7天、第15天Ang-1[(1.580±0.131)、(1.002±0.136)]、VEGF[(1.694±0.195)、(1.144±0.122)]、HIF-1αmRNA[((1.548±0.175)、(1.002±0.103)]表达与模型对照组[第7天(1.291±0.115)、(1.417±0.209)、(1.252±0.149),第15天(1.242±0.164)、(1.326±0.133)、(1.204±0.153)]比较(P<0.05)。结论:托毒生肌凝胶用于溃疡创面模型大鼠的治疗,能够加速创面的愈合,缩短愈合时间。其机制可能是通过调节Ang1/VEGF/HIF-1α信号转导通路而发挥作用。Objective:To investigate the therapeutic effect and mechanism of Tuodu Shengji gel on ulcer wound in rats.Methods:Fifty-six SD rats were selected to establish full-thickness open skin defect wounds,42 rats were selected to establish skin ulcer wound model,randomly divided into 3 groups(n=14),model control group(using blank gel),Bei Fuxin group(using Recombinant Bovine Basic Fibroblast Growth Factor Gel),Tuodu Shengji group(using Tidu Shengji gel).The remaining 14 rats were the control group.The recovery of wound in each group was observed.The number of new capillaries and the number of fibroblasts were calculated in the model rats at the treatment of 7 d and 15 d.Reverse transcription polymerase chain reaction(RT-PCR)was used to detect the expression of angiopoietin-1(Ang-1),vascular endothelial growth factor(VEGF)and hypoxia inducible factor(HIF-1α)mRNA in each group of rats at 7d and 15d.Results:The healing rate of 3d,7d and 15d in the venom group was[(41.37±16.71)%,(57.68±7.29)%,(87.29±5.74)%],which was significantly higher than that in the model control group[(21.25±19.12)%,(42.21±7.24)%,(52.18±6.28)%].The healing time of wounds in the Tuodu shengji group(16.35±2.75)d was significantly lower than that in the model control group(25.22±5.37)d and the befexin group[(20.72±4.25)d],(P<0.05).Number of new capillaries[(9.47±1.50),(4.12±1.29)]and the number of fibroblasts[(63.31±5.28),(27.98±6.03)]on 7 d and 15 d in Tuodu shengji group were compared with model control group[(7.18±1.90),(6.41±1.16),(6.41±1.16),(51.77±4.12),the difference was statistically significant(P<0.05).Expression of Ang-1[(1.580±0.131),(1.002±0.136)],VEGF[(1.694±0.195),(1.144±0.122)],HIF-1αmRNA[(1.548±0.175),(0.002±0.103)]on the 7th day and 15th day in the Tuodu shengji group compared with the model control group[on the 7th day(1.291±0.115),(1.417±0.209),(1.252±0.149)on the 15th day(1.242±0.164)、(1.326±0.133)、(1.204±0.153)](P<0.05).Conclusion:Tuodu Shengji gel is used in the treatment of ulcer wound model rats,
