高分辨CT征象应用于肺内孤立结节良恶性的鉴别诊断价值分析

摘　　要：目的:探究高分辨CT征象在肺内孤立结节良恶性鉴别中的应用价值,以找到更好的治疗肺内孤立结节患者的方法。方法:从我院于2017.1-2017.12收治的肺内孤立结节患者中,随机选择126例进行高分辨CT征象的患者进行回顾性分析,此126例患者均通过影像学检查与病理证实,观察并比较不同的高分辨CT征象在诊断肺内孤立结节良恶性中的发生率。结果:126例肺内孤立结节患者中,良性结节患者有42例,恶性结节患者有84例,在恶性结节患者的高分辨CT征象中,毛刺征、分叶征、胸膜凹陷征、血管集束征的发生率占前四位,高于良性结节患者的发病率,此差异具有统计学意义。选择血管集束征、分叶征、毛刺征的任意两个进行良恶性诊断时,鉴别敏感度为99%,特异度为85%,准确度为93%,高于单一CT征象诊断,选择三者进行联合诊断时,准确度与特异度下降,敏感度不变。结论:高分辨CT征象能够准确显示肺内孤立结节的形态,毛刺征与分叶征联合诊断可以提高对肺内孤立结节良恶性诊断的准确度、敏感度以及特异度。objective:to explore the application value of high-resolution CT in the identification of isolated nodules in the lung,and to find a better way to treat patients with solitary nodules in the lung.Methods:our hospital from 2017.1 to 2017.12 were in isolated lungs of tubercle patients,randomly selected 126 patients with high resolution CT signs were analyzed retrospectively,the 126 patients with pathologically proven by imaging examination,to observe and compare the different high resolution CT signs in diagnosing pulmonary isolated incidence of benign and malignant nodules.Results:126 cases of pulmonary nodules in patients with isolated,42 patients with benign nodules,84 patients with malignant nodules,in the high resolution CT signs of patients with malignant nodules,burr,Ye Zheng,the incidence of pleural sag indications,blood vessels,cluster sign of four,higher than the incidence of patients with benign nodules,the difference is statistically significant.Choose vessels Ye Zheng cluster characters,points,burrs of any two of the benign and malignant diagnosis and differential sensitivity was 99%,85%,the accuracy is 93%,higher than that of single sign CT diagnosis,choosing three joint diagnosis,degree of accuracy and specific fell,the sensitivity is constant.Conclusion:the high resolution CT signs can show in isolated lungs nodules form accurately,burr and points Ye Zheng joint diagnosis can improve the isolation within the lung nodules of benign and malignant diagnosis accuracy,sensitivity and specific degrees.
