含洛铂肝动脉化疗栓塞术治疗原发性肝癌临床研究

摘　　要：目的:探讨含洛铂的肝动脉化疗栓塞术在治疗原发性肝癌的疗效及其应用价值。方法:选择不可手术切除原发性肝细胞癌共92例,随机分成实验组48例(含洛铂方案):洛铂+表柔比星+丝裂霉素;对照组44例:表柔比星+丝裂霉素。观察比较两组治疗有效率及预后。结果:实验组有效率为47.92%,对照组有效率为34.10%,差异具有统计学意义(P<0.05)。两组疾病控制率无统计学差异。治疗后1月实验组患者AFP浓度明显低于对照组(P<0.05)。实验组的1、2、3年生存率及中位生存期均高于对照组(P<0.05)。多因素分析提示分组(P=0.001)及门脉癌栓(P=0.000)为独立预后因素,相对危险度分别为2.124(95%可信区间1.224~4.121)及1.211(95%可信区间1.122~2.545)。结论:含洛铂的肝动脉化疗栓塞术治疗原发性肝癌临床效果显著。但仍需更多大样本临床随机对照研究进一步证实。Objective:To evaluate the clinical efficacy and practical value of lobaplatin with transarterial chemoembolisation(TACE)for the treatment of primary hepatocellular carcinoma(HCC).Methods:A total of 92 unresectable HCC patients in our hospital were enrolled in the study.48 patients received lobaplatin plus doxorubicin and mitomycin were included experimental group,and the other 44 patients received plus epirubicin and mitomycin were referred to the control group.We assess the clinical efficacy and prognosis of lobaplatin with TACE for HCC.Results:The response rate of the tumor after treatment was 47.92%for the lobaplatin group and 34.10%for the control group(P<0.05).There was no significant difference between the two groups.The concentration of AFP and median survival and 1,2,3-years survival rate in experimental group was lower than that in control group.Multivariate cox regression analysis showed group[HR,2.124(95%CI 1.224~4.121)]and portal vein embolus[HR,1.211(95%CI 1.122~2.545)]were dependent of influential factors of prognosis.Conclusion:Lobaplatin with TACE for HCC is significant clinical efficacy,but it still need more large samples of clinical randomized controlled trials to further confirm.
