摘 要:目的比较复合树脂、陶瓷嵌体对近中-面-远中(MOD)缺损牙体组织修复体界面应力分布及大小的影响。方法建立右下颌第一磨牙两种MOD缺损的三维有限元实体模型,MOD缺损为模型A、MOD缺损+箱装洞形为模型B。采用复合树脂和陶瓷嵌体对两种模型进行修复,施加垂直于半球球心的400 N压力作用于10mm直径钢球上,采用有限元分析软件ABAQUS分析模型A、B修复体界面应力(以坐标轴为参考,与受力方向相切的两个方向上的力为剪切应力1、2,垂直方向的力为拉伸应力)及其分布情况。结果同种模型状态下,复合树脂嵌体修复MOD缺损牙体组织修复体界面剪切应力1、2和拉伸应力均小于陶瓷嵌体。同种材料修复状态下,模型B牙体组织修复体界面剪切应力1、2均大于模型A,拉伸应力稍小于模型A。在力的载荷下,复合树脂、陶瓷嵌体修复的模型A、B牙体组织修复体界面剪切应力1、2和拉伸应力的分布趋势相似,均集中在牙体组织载荷附近的外边缘,牙体组织修复体界面剪切应力1、2和拉伸应力均从应力集中处向缺损底部逐渐减小。结论复合树脂、陶瓷嵌体对MOD缺损牙体组织修复体界面应力分布的影响相似,复合树脂嵌体修复MOD缺损牙体组织修复体界面剪切应力和拉伸应力均小于陶瓷嵌体。Objective To compare the influence of composite resin and ceramic inlays on tooth-restoration interfacial stress distribution and values of mesio-occlusal-distal(MOD)cavity.Methods Two types of 3 Digital-Finite Element(3D-FE)MOD cavity models of the lower right first molar were built.Model A was a MOD cavity,and model B was a MOD cavity with box form.Composite resin and ceramic inlays were used to restore the two models and a load of 400N was vertically applied on the centre of a 10 mm diameter semisphere.Finite element analysis(FEA)software ABAQUS was used to investigate the tooth-restoration interfacial distribution and values of Model A and B(taking coordinate axis as reference,the direction of shear stress 1 and 2 were tangent to the loading direction;the tensile stress was vertical to the loading direction).Results As the same type of model,shear stress 1,2 and tensile stress of models restored with composite resin were lower than those restored with ceramic.When restored with the same material,shear stress 1 and 2 of model B were higher than those of model A,and tensile stress of model B was a little bit lower than that of model A.Under the same loading,models A and B showed the similar distribution of tooth-retoration interfacial shear stress 1,2 and tensile stress when restored with composite resin and ceramic.The shear stress 1,2 and tensile stress concentrations were all along the external margin next to the loading concentration and gradually decreased to the bottom.Conclusions There were no significant difference in the stress distribution of MOD cavities restored with composite resin and ceramic inlays.However,shear and tensile stresses of MOD cavities restored with composite resin inlays were lower than those restored with ceramic inlays.
