自凝刀联合抗HPV生物蛋白辅料治疗LEEP术后HPV持续阳性的疗效分析

摘　　要：目的观察自凝刀联合抗HPV生物蛋白辅料治疗宫颈环形电切(LEEP)术后HPV持续阳性的临床疗效。方法回顾性分析2015年9月至2016年3月于我院因宫颈病变行LEEP术后4～6个月随访,HPV持续阳性者188例,年龄28～55岁。根据病情和个人治疗意愿分为四组:暂不治疗,密切观察者35例(A组);自凝刀治疗者50例(B组);抗HPV生物蛋白辅料治疗者43例(C组);自凝刀联合抗HPV生物蛋白辅料治疗者60例(D组)。记录治疗前后宫颈液基细胞学、HPV-DNA检测和疗效及不良反应。结果治疗3个月后,D组的HPV清除率明显高于A组(P<0.001)、B组(P<0.05)和C组(P<0.05)。在四组中,HPV感染合并宫颈细胞ASCUS/LSIL者分别25例、30例、29例和41例,治疗3个月后复查,D组宫颈脱落细胞学转阴率明显高于A组(P<0.001)、B组(P<0.05)和C组(P<0.05)。结论对LEEP术后高危型HPV持续阳性的患者,给予自凝刀联合抗HPV生物蛋白辅料治疗,可以有效地降低HPV持续感染的状态。Objective To observe the clinical efficacy of self coagulant knife combined with anti HPV biological protein excipient for the treatment of HPV persistent positive after LEEP(loop electrosurgical excisional procedure).Methods A retrospective analysis of 188 cases with HPV persistent positive was made.The cases were followed up for 4-6 months after LEEP due to cervical lesions in our hospital from September 2015 to March 2016.According to the condition and individual treatment wishes,they were divided into 4 groups:temporary non treatment,close observation in 35 cases(group A),self coagulating knife treatment in 50 cases(group B),anti HPV biological protein excipient treatment in 43 cases(group C),self coagulating knife combined with anti HPV biological protein excipient treatment in 60 cases(group D).Cervical fluid based cytology,HPV-DNA detection,curative effect and adverse reaction were recorded before and after treatment.Results After 3 months of treatment,the HPV clearance rate in group D was significantly higher than that in group A(P<0.001),group B(P<0.05)and group C(P<0.05).In the four groups,HPV infection combined with cervical cell ASCUS/LSIL were 25 cases,30 cases,29 cases and 41 cases,respectively.After 3 months'treatment,the negative rate of cervical cytology in group D was significantly higher than that in group A(P<0.001),group B(P<0.05)and group C(P<0.05).Conclusions For patients with persistent high risk HPV infection after LEEP,self coagulation knife combined with anti HPV biological protein excipient treatment can effectively reduce the status of HPV persistent infection.
