摘  要:目的研究轴性高度近视眼4 mm及6 mm瞳孔直径下眼球高阶像差的特点及其与眼轴长度和眼球屈光度的相关性。方法选取北京大学国际医院眼科轴性高度近视患者共30例(30眼),眼轴长度均≥26 mm,等效球镜度数≤-6. 00 D。OPDScanⅢ光学视觉质量分析仪测量4 mm及6 mm瞳孔直径下患者角膜、眼内及全眼高阶像差,应用IOL-Master测量患者眼轴长度。使用SPSS 20. 0软件进行统计学分析,采用配对样本t检验比较4 mm与6 mm瞳孔直径下角膜、眼内及全眼高阶像差的差异,Pearson相关性分析用于分析4 mm与6 mm瞳孔直径下各高阶像差与眼轴长度及等效球镜度数的相关性。结果 4 mm瞳孔直径下角膜高阶像差、眼内高阶像差及全眼高阶像差分别为(0. 125±0. 040)μm、(0. 140±0. 042)μm和(0. 136±0. 052)μm,均较6 mm瞳孔直径下的(0. 422±0. 110)μm、(0. 348±0. 101)μm、(0. 341±0. 109)μm显著降低,差异均有统计学意义(均为P <0. 001); 4 mm瞳孔直径下角膜高阶像差与全眼高阶像差相比差异无统计学意义(P=0. 193); 6 mm瞳孔直径下角膜高阶像差比全眼高阶像差显著升高(P=0. 002)。4 mm瞳孔直径、6 mm瞳孔直径下各高阶像差与等效球镜度数和眼轴长度均无显著相关性(均为P> 0. 05)。结论轴性高度近视患者6 mm瞳孔直径比4 mm瞳孔直径下角膜高阶像差、眼内高阶像差及全眼高阶像差均显著增加。4 mm瞳孔直径及6 mm瞳孔直径下各高阶像差与等效球镜度数和眼轴长度均无显著相关性。眼内高阶像差对角膜高阶像差可能有补偿作用,眼内球差在角膜高阶像差的补偿中可能起重要作用。Objective To investigate the characteristics of high order aberrations in axial high myopia patients under 4 mm and 6 mm diameter of the pupil,and the correlation of high order aberrations and the axial length and the refractions of the eye.Methods Thirty patients(30 eyes)with axial high myopia were selected in the department of ophthalmology,Peking University International Hospital,with axis length≥26 mm and equivalent spherical refractions≤-6.0 D.High order aberrations of the cornea,intraocular and the whole eye were measured by OPD-ScanⅢoptical visual quality analyzer under 4 mm and 6 mm diameters of pupil.The axial length was measured by using the IOL-Master.All analyses were conducted by using SPSS version 20.0.Paired sample t-test was used to compare the high order aberrations between cornea,intraocular and the whole eye under 4 mm and 6 mm diameters of pupil.Pearson correlation analysis was used to analyze the correlation between high order aberrations and axial length and equivalent spherical refractions under 4 mm and 6 mm diameters of pupil.Results The corneal,intraocular and the whole eye high order aberrations of the 4 mm pupil were(0.125±0.040)μm,(0.140±0.042)μm and(0.136±0.052)μm,respectively,which were significantly higher than those of the cornea,intraocular and the whole eye high order aberrations under the 6 mm pupil(0.422±0.110)μm,(0.348±0.101)μm and(0.341±0.109)μm.The difference was statistically significant(all P<0.001);there were no significant difference between the corneal high order aberrationsand the whole eye under 4 mm diameter of pupil(P=0.193).The corneal high order aberrations were significantly higher than the total ocular under 6 mm diameter of pupil(P=0.002).Neither 4 mm diameter of pupil nor 6 mm diameter of pupil,the high order aberrations of the ocular were not significantly correlated with equivalent spherical refractions and axial length(all P>0.05).Conclusion High order aberrations of the cornea,intraocular and total ocular in axial high myopia are signifi
