弥漫性大B细胞淋巴瘤组织Livin和Caspase-3表达变化及意义

摘　　要：目的探讨组织凋亡抑制蛋白Livin和天冬氨酸特异性半胱氨酸蛋白酶3(Caspase-3)在弥漫性大B细胞淋巴瘤(DLBCL)发生、发展中的作用。方法选择DLBCL患者69例(DLBCL组)、反应性增生性淋巴结炎(RHL)患者30例(RHL组),分别取两组病变淋巴组织标本,采用免疫组化法检测Livin、Caspase-3表达。分析DLBCL组织Livin、Caspase-3阳性表达与患者临床病理参数和预后的关系。结果 DLBCL组病变淋巴组织Livin阳性表达率高于RHL组,Caspase-3阳性表达率低于RHL组,两组比较P均<0. 05。GCB型DLBCL患者病变淋巴组织Livin阳性表达率低于非GCB型DLBCL患者,Caspase-3阳性表达率高于非GCB型DLBCL患者(P均<0. 01);Ann Arbor分期为Ⅰ、Ⅱ期者病变淋巴组织Livin阳性表达率低于Ⅲ、Ⅳ期者,Caspase-3阳性表达率高于Ⅲ、Ⅳ期者(P <0. 05)。DLBCL病变淋巴组织Livin阳性表达与Caspase-3阳性表达呈负相关关系(r=-0. 308,P <0. 01)。DLBCL患者Livin阴性表达者生存率高于Livin阳性表达者,生存时间长于Livin阳性表达者(P均<0. 01); DLBCL患者Caspase-3阳性表达者生存率高于Caspase-3阴性表达者,生存时间长于Caspase-3阴性表达者(P均<0. 01)。结论 DLBCL病变淋巴组织Livin表达上调、Caspase-3表达下调,二者表达变化共同参与DLBCL的发生、发展,并与患者不良预后密切相关。Objective To investigate the expression and significance of inhibitor of apoptosis protein Livin and Caspase-3 in the diffuse large B-cell lymphoma(DLBCL).Methods Sixty-nine patients with DLBCL(DLBCL group)and 30 patients with reactive hyperplasitic lymphadenitis(RHL group)were selected.The samples of cancerous lymphoid tissues and normal lymphoid tissues were taken,respectively.Immunohistochemical method was used to detect the expres-sion of Livin and Caspase-3.The relationship between the expression of Livin and Caspase-3 in DLBCL tissues and the clin-icopathological features and prognosis of the patients was analyzed.Results The positive expression rate of Livin in the lesion lymphoid tissues of the DLBCL group was higher than that of the control group,and the positive expression rate of Caspase-3 was lower than that of the control group(P<0.05).The positive expression rate of Livin in the lesion lymphoid tissues of DLBCL patients of GCB type was lower than that of non-GCB patients,and the positive expression rate of Caspase-3 was higher than that of non-GCB patients.The positive expression rate of Livin in the lymphoid tissues of patients with stage I and I of Ann Arbor was lower than that of non-GCB type,and the positive expression rate of Caspase-3 was higher than that of non-GCB type(all P<0.05).The expression of Livin in the lesion lymphoid tissues of DLBCL patients was negatively correlated with Caspase-3 expression(r=-0.308,P<0.01).The survival rate and survival time of pa-tients with negative expression of Livin was higher than that of positive patients(P<0.01).The survival rate and survival time of patients with positive Caspase-3 expression was higher than that with negative Caspase-3 expression(P<0.01).Conclusion Livin expression is up-regulated and Caspase-3 expression is down-regulated in the lesion lymphoid tissues of DLBCL patients,and the change of Livin expression and Caspase-3 expression is involved in the occurrence and develop-ment of DLBCL and is closely related to the prognosis of patient
