摘 要:目的分析程序性死亡配体-1(PD-L1)在食管鳞癌中的表达及其临床意义。方法采用免疫组织化学染色、Western blot检测PD-L1在50例食管鳞癌患者的癌组织及其相应癌旁组织中的表达,并分析PD-L1表达与食管鳞癌临床病理学特征及预后的相关性。结果 PD-L1在食管鳞癌组织中阳性表达率为42.0%(21/50)高于癌旁组织22.0%(11/50),差异有统计学意义(P <0.05)。PD-L1蛋白的表达水平与患者肿瘤浸润深度及临床分期相关(P <0.05)。Kaplan-Meier生存分析表明PD-L1的阳性组较阴性组预后较差,PD-L1阳性组和阴性组患者30个月生存率分别为38.1%和86.2%,差异有统计学意义(P <0.05)。多变量COX回归分析结果显示PD-L1是食管癌患者的预后因素。结论 PD-L1蛋白高表达在食管鳞癌的发生发展过程中具有重要作用,可能成为食管鳞癌预后的评价指标,并为食管鳞癌的治疗提供新的靶点。Objective To investigate expression of PD-L1 and its clinical significance in patients with esophageal squamous cell carcinoma(ESCC).Methods Expression of PD-L1 protein in 50 cases of tumor and matched adjacent tissues was identified by immunohistochemical staining and Western blot.Relationship between expression and clinical characteristics was analyzed.Results The positive expression rate of PD-L1 protein was increased in cancer tissue when compared with that in adjacent noncancerous mucosa tissues(42.0 vs 22.0%,P<0.05).The expression of PD-L1 protein was closely associated with depth of infiltration and TNM stage(P<0.05).Kaplan-Meier survival analysis in a 30-months follow-up study showed that patients in PD-L1 positively expressed group experienced lower survival rate when compared with those in PD-L1 negatively expressed group(38.1%vs 86.2%,P<0.05).COX regression analysis result indicated that PD-L1 was the independent prognosis factor of ESCC.Conclusions PD-L1 plays an important role in carcinogenesis of ESCC,PD-L1 may be a potential prognostic and therapeutic target of ESCC.
