摘 要:目的应用静息态功能磁共振成像(resting-state functional magnetic resonance imaging,rs-fMRI)低频振幅(amplitude of low frequency fluctuation,ALFF)方法,研究文拉法辛对成人大脑运动皮质区基线脑活动变化的影响,探讨该药对健康成人男性运动功能的影响。方法入选8例健康成年男性,分为组A和组B,组A受试者先服用文拉法辛胶囊1粒/d,连续服用7 d后,停用3 d接着服用7 d的安慰剂1粒/d;组B受试者服药秩序与组A相反。受试者在干预前、服文拉法辛7 d后、及服安慰剂7 d后分别进行静息态功能磁共振成像低频振幅(ALFF)及食指轻叩试验检查,以比较8例受试者在不同阶段的上肢远端运动的灵活性,以及运动皮质区ALFF的改变,并分析运动皮质区存在显著差异脑区的ALFF值与运动功能的相关性。结果 8例入选受试者完成了整个实验过程;文拉法辛胶囊较安慰剂能显著改善受试者食指轻叩的执行能力;与服用安慰剂相比,服用文拉法辛胶囊后健康成人皮质运动区ALFF值升高区域主要位于双侧初级运动区、辅助运动区及双侧顶叶皮质,其中初级运动区的ALFF值的增高与对侧食指轻叩实验的执行能力改善存在正相关(person相关,r左手=0.82,P <0.01;r右手=0.88,P <0.01)。结论文拉法辛胶囊能调节运动皮质区神经元活动的兴奋性,提高运动效率,这为研究文拉法辛胶囊对脑损伤后运动功能的康复治疗提供了理论依据。Objective In this study,we explore the effects of Venlafaxin on motor function in healthy adult men with resting-state functional magnetic resonance imaging(rs-fMRI).Methods There were eight healthy men volunteers recruited in this trial who were diveded into group A and group B.Each volunteer in group A received 1 venlafaxine capsule daily in the first week,after a wash-out period of 3 days,then received 1 placebo capsule daily for a week.In contrast,Each volunteer in group B received the drug in the opposite order to group A.Respectively,before and after the administration of placebo or venlafaxin,The the volunteers were asked to execute motor tasks and accepted laboratory tests and rs-fMRI examination.Subsequently,Pearson’s correlation analyses were performed between abnormal amplitude of low frequency fluctuation(ALFF)in the brian motor cortex and the improvement of the contralateral index finger tapping.Results After administration of venlafaxin,the performance of the finger tapping test of the both hands showed a significant improvement compared to that after administration of palacebo.Compared with placebo group,venlafaxine group exhibited high ALFF in the bilateral Primary sensorimotor cortex(S1M1),supplementary motor area(SMA)and parietal.Subsequently,There there is a positive correlation between the difference of ALFF in the S1M1 and the improvement of the contralateral index finger tapping.Conclusion The findings suggest that venlafaxine can improve the exercise efficiency,regulating cortical excitability,which provides evidence for the study of rehabilitation of motor function after brain injury.
