硬脊膜动静脉瘘15例临床特征分析

摘　　要：目的探讨硬脊膜动静脉瘘(SDAVF)的临床特征,提高早期诊断率。方法选取我院收治的15例患者,对其临床表现、影像学特点等资料进行回顾性分析。结果 15例患者中以运动障碍为首发表现的有8例,感觉障碍9例,大小便障碍3例,头痛4例。15例患者中9例行脊髓MRI或MRA检查,均表现为长节段脊髓髓内病变,脊髓髓周见虫蚀样异常信号影。所有患者均接受了DSA检查。15例患者中手术治疗12例,大部分患者预后良好。结论虽然SDAVF发病率不高,但容易被漏诊或误诊,临床医生应加强对该病的认识,尽早给予恰当的治疗。Objective Objective To investigate the clinical features of Spinal dural arteriovenous fistula(SDAVF),strive for early and correct diagnosis.Method A retrospective analysis of 15 cases of SDAVF clinical and radiological manifestation was performed.Result The initial symptoms and signs were concentrated in movement disorders in 8 of 15cases,9 cases with sensory disturbances,3 cases with bowel and bladder dysfunction,4 cases with headache.Spinal MRI or MRA were performed in 9 of 15 cases,which showed the swelling and abnormal signal in the spinal cord lesion.There were“moth-eaten”flow void phenomena in the spinal space.Spinal angiography was underwent in all the patients to verify the diagnosis.12 of 15 patients accepted therapy of surgery,most cases improved immediately after treatment.Conclusion Although SDAVF is a rare disease,it is easy to be misdiagnosed,we should strengthen the recognition of this disease,and choose proper treatment methods.
