上皮性卵巢癌中V-ATPase与Ki-67的表达及其临床意义

摘　　要：目的探讨液泡膜-ATP酶(V-ATPase)及Ki-67在上皮性卵巢癌(EOC)的表达、临床意义及二者的相关性。方法免疫组织化学染色法检测EOC及正常卵巢上皮组织中VATPase及Ki-67蛋白的表达,分析两者相关性及V-ATPase与EOC临床病理特征、肿瘤发展的关系。结果 V-ATPase蛋白在EOC组织阳性表达率与正常卵巢上皮细胞组织阳性表达率相比(61. 3%vs 12. 5%),差异有统计学意义(P <0. 05)。V-ATPase表达与EOC患者的年龄、病理类型、脉管浸润无明显相关性(P> 0. 05),而与卵巢癌FIGO分期(P=0. 002)、病理分级(P=0. 001)、淋巴结转移(P=0. 020)以及肿瘤大小(P=0. 015)有明显相关性(P <0. 05)。且在EOC中,V-ATPase和Ki-67阳性表达呈正相关性(rs=0. 238,P<0. 05)。结论 V-ATPase在EOC组织中高表达。V-ATPase和Ki-67可能共同参与EOC肿瘤的发生发展,二者结合检测有可能对卵巢恶性肿瘤的临床诊断和治疗有一定价值。Objective To investigate the expression and clinical significance of vacuolar membrane-ATPase(V-ATPase)and Ki-67 in epithelial ovarian cancer(EOC).Methods The expression of V-ATPase and Ki-67 protein in EOC and epithelial ovarian tissues were detected by inmmunohistochemistry method,their relationship was evaluated,and the correlation between V-ATPase and clinicolpathological features and tumor progression were analyzed.Results The positive expression of V-ATPase was 61.3%in patients with EOC,significant higher than that in epithelial ovarian tissues(12.5%).There was no significant correlation between V-ATPase expression and age,pathological type,and vascular invasion in patients with epithelial ovarian cancer(P>0.05).Expression of V-ATPase was significant associated with clinical stage(P=0.002),tumor grade(P=0.001),lymph node metastasis(P=0.020)and tumor size(P=0.015).In EOC,the positive expression of V-ATPase was closely related with Ki-67(r s=0.238,P<0.05).There was a positive correlation between the expression of V-ATPase and Ki-67(r s=0.238,P<0.05).Conclusion The expression of V-ATPase is up-regulated in EOC tissues.V-ATPase and Ki-67 may be involved in the evolution and progression of EOC.Combination detection of V-ATPase and Ki-67 may play an important role in the clinical diagnosis and treatment of malignant EOC.
