摘 要:目的探讨多囊卵巢综合征(PCOS)患者卵泡液特异性代谢指标及其对体外受精-胚胎移植(IVF-ET)妊娠结局的影响。方法选自2016年6月至2017年8月我院85例因输卵管因素或男方因素致女性不孕者为正常对照组,80例PCOS患者为PCOS组。通过分析卵泡液多种氨基酸、有机酸和脂肪酸,获得PCOS潜在的生物标志物。并经IVT-ET观察两组获卵数、正常受精率、优质胚胎率、胚胎着床率、临床妊娠率妊娠结局指标。结果 PCOS组卵泡液4种氨基酸明显低于对照组,差异有统计学意义(P <0.05);PCOS组1种氨基酸、6种有机酸和6种脂肪酸高于对照组,差异有统计学意义(P <0.05);PCOS组获卵数、正常受精率、临床妊娠率显著低于对照组,差异有统计学意义(P <0.05),胚胎着床率两组差异无统计学意义(P> 0.05)。结论初步获得PCOS患者卵泡液伴有氨基酸、有机酸及脂肪酸特异性代谢指标,卵泡液中特异性代谢物可能是影响PCOS患者IVF-ET妊娠结局重要因素。Objective To investigate specific metabolic markers in follicular fluid and its effect on preg-nancy outcomes of in vitro fertilization and embryo transfer(IVF-ET)in patients with polycystic ovary syndrome(PCOS).Methods 85 cases of female infertility due to fallopian tube factor or male factor as the control group and 80 cases of PCOS patients for the study group in our hospital from June 2016 to August 2017.The potential biomarker of PCOS was obtained by analyzing follicular fluids amino acids,organic acids and fatty acids between the PCOS group and the control group.The number of retrieved oocytes,rates of normal fertilization and high-quality embryos,implantation rate of embryos,clinical pregnancy rate,and pregnancy outcomes were measured.Results Four types of amino acid were significantly lower in the PCOS group than in the control group(P<0.05),One type of amino acids,six types of organic acids and six types of fatty acids were higher in the PCOS group than in the control group(P<0.05).The number of eggs,normal fertilization rate and clinical pregnancy rate in the PCOS group were significantly lower than those in the control group(P<0.05),but there was no significant difference in embryo implantation rate between the two groups(P>0.05).Conclusion Follicular fluid in patients with PCOS was initially associated with specific metabolic parameters of amino acids,organic acids and fatty acids.Specific metabolites in follicular fluid may be an important factor affecting pregnancy outcome of IVF-ET in patients with PCOS.Metabolites of amino acids,fatty acids and organic acids of follicular fluid are important biomarkers for the quality of oocytes in patients with PCOS,which can provide the basis for further exploring the influencing factors of oocyte quality.
