摘  要:目的探讨双能量成像指标、微血管密度与实性肺腺癌分化程度和淋巴结转移的相关性。方法术前行CT双能成像且经手术切除病理学检查证实为实性肺腺癌的患者43例,术后检测肿瘤组织微血管密度(MVD)数值。实性肺腺癌是否转移组双能量各指标等的比较应用t检验。不同分化程度实性肺腺癌能谱曲线斜率比较采用单因素方差分析,用Pearson相关性分析评价各双能量指标与MVD的关系。结果实性肺腺癌大小、碘浓度、标准化碘浓度(NIC)、动脉期斜率和MVD在有无淋巴结转移组间差异均有统计学意义(P <0.05);碘浓度、NIC、动脉期斜率和MVD在不同分化程度之间差异有统计学意义(P <0.05),碘浓度、NIC、动脉期斜率和MVD在高分化和低分化组间差异有统计学意义(P <0.05);肿瘤大小、碘浓度、NIC、动脉期斜率与MVD均成正相关(r值分别为0.359、0.757、0.843、0.771,均P <0.05)结论双源CT双能量成像中的碘浓度、NIC、动脉期斜率可作为术前判断实性肺腺癌淋巴结转移及分化程度的重要预测指标之一。Objective To investigate the correlations among dual-energy imaging indices,microvessel density(MVD)and differentiation of solid lung adenocarcinoma and lymph node metastasis.Methods Forty-three patients with solid lung adenocarcinoma confirmed by preoperative CT dual-energy imaging and surgical resection pathology were examined for the postoperative MVD values.The groups with or without the lymphnodes metastasis of solid lung adenocarcinoma were compared using a t-test method.The slopes of energy spectrum curves with differ-ent differentiation degrees were analyzed by One-Way ANOVA method.Pearson correlation analysis was used to evaluate the relationship among each dual-energy index,solid lung adenocarcinoma and MVD.Results There were significant differences in the size,iodine concentration,normalized iodine concentration(NIC),arterial phase slope and MVD of lymph node metastasis between groups with or without the lymphnodes metastasis(P<0.05,respectively).Iodine concentration,NIC,arterial slope and MVD were differently differentiated.Significant differences were observed among differentiation grades(P<0.05).The iodine concentration,NIC,arterial phase slope and MVD were significantly different between the highly differentiated and the poorly differentiated groups(P<0.05).Tumor size,iodine concentration,NIC Both arterial phase slope and MVD were positively correlated(r value was 0.359,0.757,0.843,0.771;P<0.05,respectively).Conclusion Iodine concentration,NIC and arterial phase slope in dual-source CT dual-energy imaging can be used as one of the important predictors for preop-erative lymph node metastasis and differentiation in solid lung adenocarcinoma.
