摘  要:目的:探讨口腔扁平苔藓(oral lichen planus,OLP)病损组织中Th17细胞与调节性T细胞(Treg)的平衡状态(Th17/Treg)在OLP病损形成中的作用和意义。方法:纳入41例OLP病损组织和15例正常口腔黏膜组织作为对照。采用免疫组化法检测IL-17、FOXP3和TGF-β1蛋白表达及分布,实时荧光定量PCR法(real-time fluorescent quantitative PCR,q PCR)检测IL-17、RORc和FOXP3 mRNA的表达。结果:OLP中IL-17、FOXP3和TGF-β1的阳性表达率(85. 37%、87. 80%和82. 93%)均显著高于对照组(20. 00%、13. 33%和13. 33%)(P <0. 01)。IL-17、RORc、FOXP3 mRNA的相对表达量也均高于对照组(P <0. 05)。糜烂型OLP中RORc/FOXP3比值(2. 05±1. 44)高于对照组(1. 02±0. 52)(P <0. 05),但非糜烂型OLP(1. 30±1. 14)与对照组相比差异无统计学意义。Spearman相关性分析显示RORc与FOXP3 mRNA的相对表达量呈正相关(r=0. 69,P <0. 05)。结论:Th17及Treg细胞均与OLP病损形成有关,其中Th17/Treg失衡在糜烂型OLP病损形成中可能起到一定作用。Objective:To investigate Th17/Treg balance and its role and in the development of oral lichen planus(OLP)lesion.Methods:The oral mucosa tissues of 41 patients with OLP(21 with non-erosive OLP and 20 patients with erosive OLP)and 15 healthy controls were recruited.The protein expression and distribution of IL-17,FOXP3 and TGF-β1 were detected by immunohistochemistry(IHC).Real-time fluorescence quantitative PCR(qPCR)technique was used to detect the mRNA expression of IL-17,RORc and FOXP3.Results:The positive protein expression of IL-17(85.37%),FOXP3(87.80%)and TGF-β1(82.93%)in OLP lesions were significantly higher than that in normal oral mucosa(20.00%,13.33%and 13.33%)(P<0.01).The mRNA expression of IL-17,RORc and FOXP3 in OLP tissue was higher than those in the control(P<0.05).It was found that RORc/FOXP3 ratio was increased in erosive OLP lesion compared with the control,nevertheless,there was no significant correlation was observed between the non-erosive OLP and control group.The Spearman correlation analysis showed that RORc mRNA expression was positively correlated with FOXP3mRNA expression.Conclusion:Th17 and Treg cells are both related with the formation of OLP lesions.Th17/Treg imbalance may play a role in the development of erosive OLP.
