摘 要:目的:应用三维激光扫描技术与头影测量评价Invisalign矫治器远移上颌磨牙的临床疗效。方法:纳入22位应用Invisalign远移上颌磨牙的成年患者,收集患者磨牙远移前后的模型与头颅侧位片,应用3_Shape Trios 3扫描仪扫描模型并重建三维数字化模型,通过三维模型重叠测量目标牙在三维方向的变化,并与Clin Check预期移动量进行比较,分析头颅侧位片骨性、牙性改变。结果:三维模型重叠得出上颌第一磨牙实际远移量为(2. 04±0. 92) mm,与预期值(3. 16±1. 08) mm相比,表达率为64. 56%,支抗牙有少量支抗丢失。头影测量结果显示下颌平面角变化无统计学差异。结论:Invisalign矫治器推磨牙远移效果较好,下颌平面角无增加,但预计表达率有待提高,支抗牙有部分支抗丢失,提示临床医生增加支抗控制及增加预设量来达到需要量。Objective:To evaluate the clinical efficacy of Invisalign on distalizing maxillary molars using three-dimensional laser scanner technology and cephalogram.Methods:22 non-growing subjects who needed Maxillary molars distalization with Invisalign were selected.Plaster cast models and cephalograms were taken before and after distalization.The three-dimensional digital models were created from a 3_Shape Trios 3 laser scanner.The actual movement of the maxillary first molar,first and second premolar and central incisor on three-dimension was measured using Rapidform 2006 software and compared with the predicted movement from ClinCheck.Cephalograms about dental and skeletal changes were also measured.Results:The first maxillary molar showed significant difference between the actual and predicted movement posteriorly on three-dimensional digital model measurements.The first maxillary molar actually moved distally by(2.04±0.92)mm,the predicted movement distally was by(3.16±1.08)mm,and the efficacy was 64.56%.The premolars and central incisor had slight anchorage loss.The mandibular plane angle showed no significant difference between pre-and post-distalization cephalograms.Conclusion:Invisalign appliance can distalize maxillary molar effectively.The mandibular plane angle dose not increase.Meanwhile,there may be anchorage loss in anchorage teeth,anchorage control and predicted increase changes should be emphasized.
