自锁托槽矫治器应用于牙周病患者正畸治疗对牙周指数及龈沟液中IL-1β和TNF-α表达的影响

摘　　要：目的:观察自锁托槽矫治器治疗牙周病正畸治疗患者疗效及对龈沟液中IL-1β和TNF-α表达的影响。方法:纳入40例牙周病患者,随机分为2组(n=20),观察组采取自锁托槽矫正器治疗,对照组采取传统托槽矫正器治疗。比较2组菌斑指数(PLI)、牙龈指数(GI)、龈沟出血指数(SBI)及治疗前后龈沟液(GCF)量及其中IL-1β、TNF-α水平。结果:与对照组比较,观察组治疗4周、8周PLI、GI及SBI均降低(P <0. 05),观察组治疗后1、4、8周GCF量均更低(P <0. 05);与治疗前比较,2组治疗后1、4、8周GCF量均升高(P <0. 05)。与对照组比较,观察组治疗后1、4、8周IL-1β水平均更低(P <0. 05);与治疗前比较,2组治疗后1、4、8周IL-1β水平均升高(P <0. 05),与对照组比较,观察组治疗后1、4、8周TNF-α水平均更低(P <0. 05);与治疗前比较,2组治疗后1、4、8周TNF-α水平均升高(P <0. 05)。结论:自锁托槽矫治器可降低牙周指数及龈沟液中IL-1β和TNF-α水平。Objective:To observe the effects of self ligating appliance used in orthodontic treatment of patients with periodontal disease on the periodontal index and the expression of IL-1βand TNF-αin gingival crevicular fluid(GCF).Methods:40 cases with periodontal disease underwent fixed orthodontic treatment were randomly divided into 2 groups(n=20)and treated by self ligating appliance(test group)and traditional orthodontic appliance(control group)respectively.GCF and IL-1βand TNF-αlevels in GCF,plaque index(PLI),gingival index(GI)and sulcus bleeding index(SBI)were measured and compared statistically.Results:Compared with the control group,the PLI,GI and SBI were dereased in the test group 4 weeks and 8 weeks after treatment(P<0.05),GCF in test group was less than that in the control group 1,4 and 8 weeks after treatment(P<0.05).1,4 and 8 weeks treatment GCF of the 2 groups increased(P<0.05).Compared with the control group,IL-1βin the test group were lower 1,4 and 8 after treatment(P<0.05).IL-1βin the test group increased 1,4 and 8 weeks after treatment(P<0.05).Compared with the control group,the TNF-αin the test group was lower 1,4 and 8 weeks after treatment(P<0.05).TNF-αincreased in the 2 groups 1,4 and 8 weeks after treatment(P<0.05).Conclusion:Self ligating applicance can effectively reduce PLI,GI,SBI,GCF and the IL-1βand TNF-αlevel in GCF
