摘  要:目的:利用CBCT探讨影响成人上中切牙区不同部位牙槽骨厚度的因素。方法:100例18~35岁患者(女55例,男45例,平均年龄26. 6岁),常规CBCT拍摄,使用Mimics 15. 0软件定点测量左侧上中切牙矢状截面根尖、根中、牙槽嵴顶处牙槽骨厚度,Dolphin 11. 0软件头影测量ANB、Wit’s并行矢状骨面型分类,SN-MP、FHI(S-Go/N-Me)行垂直骨面型分类。利用SPSS 13. 0软件多元线性回归分析牙槽骨厚度与性别、矢状骨面型、垂直骨面型的相关性,独立样本t检验进一步比较不同亚组间的差异。结果:上中切牙区牙槽骨厚度在舌侧根中及根尖牙槽骨厚度、根中及根尖牙槽骨总厚度男性均大于女性(P <0. 05);垂直生长型较平均生长型牙槽骨薄,在唇侧根中及根中总厚处中均有显著差异(P <0. 05);在矢状骨面型唇侧根中及根中总厚度处,骨Ⅰ类牙槽骨厚度大于骨Ⅱ类(P <0. 05)。结论:成人上中切牙区槽骨厚度受性别、垂直骨面型和矢状骨面型不同程度影响,相较于女性、高角人群,男性、均角患者前切牙内收相对安全,且同条件下骨Ⅱ类较Ⅰ类根中唇侧骨板薄。Objective:To study the related factors which influence the upper incisor alveolar thickness of adults in Xi’an using CBCT.Methods:CBCT data of 100 adult patients(18-35 years old with a mean age of 26.6 years)including 45 males and 55 females were imported into Mimics 15.0 software to obtain screenshots of the sagittal sections of the maxillary left central incisor.At the level of apical,middle root and crest of ridge,the alveolar thickness in labial,lingual side and the total was measured,then patients were classified into skeletal Class I and II according to ANB and Wit’s,and correspondingly into high and average angle according to SN-MP and FHI(S-Go/N-Me)by importing into Dolphin 11.0.Finally,the relevance of each measurement to gender,sagittal facial type,vertical facial type was analyzed via multiple linear regression and specific subgroups discrepancies using independent sample t test.Results:The lingual alveolar was significantly thicker in the males than in the females at the level of apical,middle root and the total alveolar thickness in middle and apical root(P<0.05).The thickness was significantly related to vertical facial type,and compared with average angle type,high angle type had a thinner alveolar,mainly reflected at the middle of root(P<0.05).The incisor alveolar bone thickness was also significantly related to sagittal facial type,and at the middle of the root in labial side and the whole thickness,was thicker in skeletal class I than in class II(P<0.05).Conclusion:The thickness of adult alveolar on the central incisor area can be affected by gender,sagittal and vertical facial type.Skeletal class II subjects possess a thinner alveolar bone in the labial side than skeletal class I.
