摘 要:目的探讨川芎嗪注射液对重型急性胰腺炎(SAP)大鼠肾损伤的影响及其作用机制。方法将45只雌性SD大鼠随机分为正常组、模型组、川芎嗪组,模型组、川芎嗪组大鼠均予以4%牛磺胆酸钠经腹逆行胰胆管注射制作SAP模型,川芎嗪组大鼠在造模术后10 min均经尾静脉注射0. 6%川芎嗪注射液10 m L/kg。检测3组大鼠术后12 h、24 h和48 h血清淀粉酶(AMY)、尿素氮(BUN)和肌酐(Cr)水平,HE染色观察3组大鼠肾脏实质组织病理变化,TUNEL法检测肾组织细胞凋亡情况,Western Blot法检测肾组织中Bcl-2及Bax蛋白表达情况。结果模型组和川芎嗪组大鼠各时间血清AMY、BUN、Cr水平均明显高于正常组(P均<0. 05),且随着时间延长,模型组和川芎嗪组大鼠血清AMY、BUN、Cr水平均显著提高(P均<0. 05),但川芎嗪组均明显低于模型组(P均<0. 05)。川芎嗪组各时间点肾脏组织损伤程度较模型组轻,细胞凋亡指数明显低于模型组(P均<0. 05),Bcl-2/Bax值明显高于模型组(P均<0. 05)。结论川芎嗪可能通过提高Bcl-2蛋白表达量、降低Bax蛋白表达量以抑制SAP大鼠肾小管上皮细胞凋亡,从而发挥对肾脏的保护作用。Objective It is to investigate the effect of ligustrazine injection on renal injury in rats with severe acute pancreatitis(SAP)and to explore its mechanism.Methods Forty-five female Sprague-Dawley rats were randomly divided into normal group,model group and ligustrazine group.The rats in the model group and ligustrazine group were treated with 4%sodium taurocholate for retroperitoneal cholangiopancreatography to establish SAP models.The rats in the ligustrazine group were injected with 0.6%ligustrazine injection 10 mL/kg 10 min from tail vein after the models were established.The levels of serum amylase(AMY),urea nitrogen(BUN)and creatinine(Cr)were measured at 12 h,24 h and 48 h after operation in the rats of the three groups.The pathological changes of renal parenchyma were observed by HE staining.Renal tissue cells were detected by TUNEL.The expression of Bcl-2 and Bax protein in renal tissue was detected by Western Blot method.Results The serum levels of AMY,BUN and Cr in the model group and the ligustrazine group were significantly higher than those in the normal group at every time point(P<0.05),and the levels of serum AMY,BUN and Cr in the model group and the ligustrazine group were significantly increased with time(P<0.05),but the levels in the ligustrazine group was significantly lower than that in the model group(P<0.05).The degree of renal tissue damage was lower in the Ligustrazine group than that in the model group,while the apoptotic index was significantly lower than that in the model group(P<0.05),and the Bcl-2/Bax value was significantly higher than that in the model group(P<0.05).Conclusion Ligustrazine may inhibit the apoptosis of renal tubular epithelial cells in SAP rats by increasing the expression of Bcl-2 protein and decreasing the expression of Bax protein.
