光学相干断层扫描和视野检查在青光眼诊断中的应用

摘  要:目的探讨光学相干断层扫描(OCT)和视野检查在早期青光眼诊断中的应用价值。方法选取正常人30眼,青光眼患者40眼(其中原发性开角型青光眼24眼,原发性闭角型青光眼16眼),分别对各组采集的视盘参数,神经纤维层(RNFL)厚度参数进行比较;对光学相干断层扫描和视野检查的诊断阳性率进行对比分析。结果青光眼各组上方、下方、鼻侧、颞侧各象限及平均RNFL厚度均明显低于正常组。青光眼各组与正常组的DA无明显差异(P>0.05),但CA、CV、C/D、水平C/D、垂直C/D、RA、RV均差异显著(P<0.01)。自动视野检查结果青光眼视野缺损阳性率为80.0%,视盘≥0.5为87.5%,神经纤维层厚度受损率为92.5%。结论 RNFL厚度、视盘参数、视野检查的联合应用,有利于青光眼的早期诊断。Objective To evaluate the value of optical coherence tomography(OCT)and standard automatic perimetry(SAP)in early glaucoma diagnosis.Methods 30 normal eyes and 40 glaucoma eyes(24 primary open angle glaucoma and 16 primary closed angle glaucoma)were compared in the parameters of optic disc and retinal nerve fiber layer(RNFL).Comparative analysis of diagnostic positive rate of optical coherence tomography and standard automatic perimetry were done.Results The t hickness o f t he u pper,l ower,n asal a nd t emporal q uadrants and mean RNFL were significantly lower than those of normal group.There was no significant difference in DA between glaucoma groups and normal group(P>0.05),but there were significant differences in CA,CV,C/D,level C/D,vertical C/D,RA,RV(P<0.01).The positive rates of visual field defect in standard automatic perimetry were 80.0%,87.5%for optic disc≥0.5 and 92.5%for thickness of retinal nerve fiber layer.Conclusion The combined application of RNFL thickness,optic disc parameters and standard automatic perimetry is helpful for early diagnosis of glaucoma.
