左心室血管肉瘤超声误诊为左心室血栓回顾分析

摘　　要：目的探讨左心室血管肉瘤发病机制及其左心室血管肉瘤误诊为左心室血栓的原因、超声表现。方法对误诊为左心室血栓1例的临床资料进行回顾性分析。结果患者因胸痛、胸闷4年余于饮酒时突发胸痛,胸闷,考虑为"急性广泛前壁心肌梗死",在我院予行左冠PTCA及支架植入术。再次就诊我院,完善相关检查。心电图提示急性下壁心梗,CDFI:其内未见明显血流信号,超声提示:左室壁节段性运动减弱;左室轻度扩大;左室心尖部低回声团;左室收缩及舒张功能减退。结论临床罕见、惯性思维诊断及鉴别诊断不足,导致本例误诊的主要原因。加强对该病认识、综合全面分析病情,尽早检查螺旋CT、PETCT和MRI,为临床手术治疗提供可靠依据,延长患者生命。Objective To investigate the pathogenesis of left ventricular angiosarcoma and the causes of misdiagnosis of left ventricular hemangiosarcoma as left ventricular thrombosis.Methods The clinical data of 1 case misdiagnosed as left ventricular thrombus were analyzed retrospectively.Results Because of chest pain,chest tightness for more than 4 years,chest pain,chest tightness,considered as"acute extensive anterior wall myocardial infarction,"left coronary PTCA and stent implantation in our hospital.He visited our hospital again and improve relevant examinations.Electrocardiogram showed acute inferior myocardial infarction,CDFI:no obvious blood flow signal was found in it.Ultrasound indicated:segmental weakening of left ventricular wall motion;slight left ventricular enlargement;left ventricular apical hypoechoic mass(considering:thrombus?)Left ventricular systolic and diastolic function decreased.Conclusion The main causes of misdiagnosis in this case are rarely seen,lack of inertial thinking and differential diagnosis.We should strengthen the understanding of this disease,analyze the disease comprehensively and comprehensively,and check the spiral CT,PETCT and MRI as early as possible.
