超声内镜在胃黏膜隆起性病变诊断及内镜微创中的应用

摘　　要：目的分析超声内镜在胃黏膜隆起性病变诊断及其内镜微创治疗中的应用价值。方法选择2016年1月—2018年3月在我院接受治疗的胃黏膜隆起性病变75例,先行CT检查,再接受超声内镜诊断及治疗,以术后病理学检查为准,分析诊断准确性及治疗效果。结果 75例胃黏膜隆起样病变病理学检查,明确胃部息肉26例,胃脂肪瘤5例,胃平滑肌瘤8例,胃间质瘤27例,胃炎增生4例,异位胰腺2例,胃癌3例;胃部病灶直径<1 cm者16例,1～2 cm者29例,> 2 cm者30例;超声内镜诊断结果与病理结果相符率92.00%,高于经CT检查的62.67%(P <0.01);超声内镜治疗成功率89.33%。结论超声内镜在胃黏膜隆起性病变中具有良好的诊断价值,而且在内镜微创治疗中的应用价值良好。Objective To analyze the application value of endoscopic ultrasonography in the diagnosis of gastric mucosal protuberant lesions and endoscopic minimally invasive treatment.Methods From January 2016 to March 2018,75 cases of gastric mucosal protuberant lesions were treated in our hospital.CT examination was performed first,and then endoscopic ultrasonography was used to diagnose and treat them.Results 75 cases of gastric mucosal protuberant pathological changes were examined,including 26 cases of gastric polyps,5 cases of gastric lipoma,8 cases of gastric leiomyoma,27 cases of gastric stromal tumors,4 cases of gastritis,2 cases of heterotopic pancreas and 3 cases of gastric cancer,the diameter of gastric lesions was fewer than 1 cm in 16 cases,1~2 cm in 29 cases,and 2 cm in 30 cases.The coincidence rate of the diagnosis of endoscopic ultrasonography and pathological results was 92%,which was significantly higher than that of CT(62.67%),P<0.01,and the success rate of endoscopic ultrasonography was 89.33%.Conclusion Endoscopic ultrasonography has a good diagnostic value in protuberant lesions of gastric mucosa,and has a good application value in endoscopic minimally invasive treatment.
